Prevalent dental manifestations in serumpositive for HIV: literature review

Resumo: Individuals with a deficiency in the immune system are more likely to develop oral lesions, which can be seen in 70-90% of patients with the Human Immunodeficiency Virus (HIV). Therefore, the objective of this study is to identify in the literature the oral manifestations prevalent in HIV-positive individuals, as well as the behavior of the dentist. This is a narrative literature review, of an explanatory nature, carried out by means of a survey in the electronic databases: PubMed, LILACS, SciELO, BVS, Scholar Google, in the period between 2010 and 2022. For the search, the Descriptors in Health Sciences (DeCS): "candidiasis", "periodontal diseases", "herpes simplex", "oral hairy leukoplakia", "kaposi's sarcoma", "non-hodgkin's lymphoma", with the association of the Boolean operators AND and OR for the terms “HIV” and “dentistry”. 42 references were selected for the study. According to the literature studied, it became evident that oral manifestations are present in patients with HIV, presenting lesions with clinical and intensity variations, which can develop according to the susceptibility of the immune system and the stage of natural evolution of the disease. These include candidiasis, herpes simplex, periodontal diseases, oral hairy leukoplakia, Kaposi's sarcoma and non-Hodgkin's lymphoma. Oral manifestations are usually the first evidence of the presence of HIV. In this way, the importance of the role of the dentist is evidenced, as this professional can act in the early diagnosis, favoring an appropriate therapy and reduction of morbidity.

Por lo tanto, el objetivo de este estudio es identificar en la literatura las manifestaciones orales prevalentes en individuos VIH positivos, así como el comportamiento del odontólogo. Se trata de una revisión bibliográfica narrativa, de carácter explicativo, realizada mediante encuesta en las bases de datos electrónicas: PubMed, LILACS, SciELO, BVS, Scholar Google, en el período comprendido entre 2010 y 2022. Para la búsqueda se utilizaron los Descriptores en Ciencias de la Salud (DeCS): "candidiasis", "enfermedades periodontales", "herpes simplex", "leucoplasia vellosa oral", "sarcoma de kaposi", "linfoma no hodgkiniano", con la asociación de los operadores booleanos AND y OR para los términos “VIH” y “odontología”. Se seleccionaron 42 referencias para el estudio. De acuerdo con la literatura estudiada, se evidenció que las manifestaciones orales están presentes en pacientes con VIH, presentando lesiones con variaciones clínicas y de intensidad, que pueden desarrollarse de acuerdo con la susceptibilidad del sistema inmunológico y el estadio de evolución natural de la enfermedad. Estos incluyen candidiasis, herpes simple, enfermedades periodontales, leucoplasia vellosa oral, sarcoma de Kaposi y linfoma no Hodgkin. Las manifestaciones orales suelen ser la primera evidencia de la presencia del VIH. De esta forma, se evidencia la importancia del papel del odontólogo, ya que este profesional puede actuar en el diagnóstico precoz, favoreciendo una adecuada terapéutica y reducción de la morbilidad.Indivíduos com deficiência no sistema imunológico são mais propensos a desenvolverem lesões orais, que podem ser observadas em 70-90% dos pacientes portadores do Vírus da Imunodeficiência Humana (HIV). Sendo assim, o objetivo deste estudo é identificar na literatura as manifestações orais prevalentes em indivíduos HIV positivos, bem como a conduta do Cirurgião-dentista. Trata-se de uma revisão de literatura narrativa, de caráter explicativo, realizada mediante levantamento nas bases de dados eletrônicas: PubMed, LILACS, SciELO, BVS, Scholar Google, no período entre 2010 e 2022. Para a busca, foram utilizados os Descritores em Ciências da Saúde (DeCS): “candidíases”, “doenças periodontais”, “herpes simples”, “leucoplasia pilosa oral”, “sarcoma de kaposi”, “linfoma não-hodgkin”, com associação dos operadores booleanos AND e OR para os termos “HIV” e “odontologia”. Foram selecionadas 42 referências para a realização do estudo. De acordo com a literatura estudada, tornou-se evidente que manifestações orais estão presentes em pacientes com HIV, apresentando lesões com variações clínicas e de intensidade, as quais podem se desenvolver de acordo com a susceptibilidade do sistema imunológico e o estágio de evolução natural da doença, destacam-se as candidíases, herpes simples, doenças periodontais, leucoplasia pilosa oral, sarcoma de Kaposi e o linfoma não-Hodgkin. As manifestações orais são em geral as primeiras evidências da presença do HIV.
