Clinical pharmacist interventions in the identification and prevention of problems related to pharmacotherapy in a tertiary teaching hospital

Resumo: | The present study aims to analyses the pharmaceutical interventions in the identification and prevention of problems related to pharmacotherapy. This study has a quantitative, transversal and descriptive approach. Data were obtained in the period from January to November 2021. All patients admitted to a University Hospital in Sergipe, within the period described above, who required pharmaceutical intervention, were included in this study. Therefore, the variables of interest analyzed were divided into three axes: I) patient data: a) age, b) sex and c) comorbidities; II) problems related to detected drugs and III) pharmaceutical interventions carried out and their acceptability. After collection, data were recorded separately in Microsoft Excel® spreadsheets and analyzed using central tendency techniques. The population studied comprised 357 patients, the majority being male (53%; n=191), with at least one comorbidity, mostly hypertension (48.4%; n=205), diabetes (27%; n=115) and dyslipidemia (6%; n=26). Most interventions (64%; n=325) were accepted and most of these were related to dose adjustment, missing medication, need for additional medication and route of administration. Of the pharmaceutical interventions that were not accepted, it was observed that 26% (n=129) of prescribers did not justify it and 10% (n=52) did it. In view of the results presented, it can be seen that most pharmaceutical interventions were accepted, which may be related to the advancement of understanding of the role of the clinical pharmacist in the health team. These changes bring benefits in clinical outcomes and pharmacoeconomics parameters, since the performance of the multidisciplinary team aims to promote adequate and safe pharmacotherapy for the patient, through the prevention of drug-related problems as well as their rational use. |

Este estudio tiene un enfoque cuantitativo, transversal y descriptivo. Los datos fueron obtenidos en el período de enero a noviembre de 2021. Fueron incluidos en este estudio todos los pacientes internados en un Hospital Universitario de Sergipe, en el período descrito anteriormente, que requirieron intervención farmacéutica. Por tanto, las variables de interés analizadas se dividieron en tres ejes: I) datos de los pacientes: a) edad, b) sexo yc) comorbilidades; II) problemas relacionados con los fármacos detectados y III) intervenciones farmacéuticas realizadas y su aceptabilidad. Después de la recolección, los datos se registraron por separado en hojas de cálculo de Microsoft Excel® y se analizaron utilizando técnicas de tendencia central. La población estudiada estuvo compuesta por 357 pacientes, siendo la mayoría del sexo masculino (53%; n=191), con al menos una comorbilidad, en su mayoría hipertensión arterial (48,4%; n=205), diabetes (27% n=115) y dislipidemia (6%; n=26). La mayoría de las intervenciones (64%; n=325) fueron aceptadas y la mayoría de ellas estaban relacionadas con el ajuste de dosis, la falta de medicación, la necesidad de medicación adicional y la vía de administración. De las intervenciones farmacéuticas que no fueron aceptadas, se observó que el 26% (n=129) de los prescriptores no lo justificaron y el 10% (n=52) lo hicieron. En vista de los resultados presentados, se puede ver que la mayoría de las intervenciones farmacéuticas fueron aceptadas, lo que puede estar relacionado con el avance en la comprensión del papel del farmacéutico clínico en el equipo de salud. Estos cambios traen beneficios en los resultados clínicos y parámetros farmacoeconómicos, ya que la actuación del equipo multidisciplinario tiene como objetivo promover una farmacoterapia adecuada y segura para el paciente, a través de la prevención de problemas relacionados con los medicamentos, así como su uso racional.O presente estudo tem como objetivo analisar as intervenções farmacêuticas com enfoque na prevenção de problemas relacionados à farmacoterapia. A maior parte das intervenções (64%; n=325) foram aceitas e dessas a maioria estava relacionada ao ajuste de dose, medicamento em falta, necessidade de medicamento adicional e via de administração. Das intervenções farmacêuticas que não foram aceitas, observou-se que 26% (n=129) dos prescritores não justificaram e 10% (n=52) justificaram. Diante dos resultados apresentados, pode-se perceber que a grande parte das intervenções farmacêuticas foram aceitas, o que pode estar relacionado com o avanço da compreensão do papel do farmacêutico clínico na equipe de saúde.
