Outlining the pharmacotherapeutic profile and assessment of adherence to treatment in hypertensive and/or diabetic patients: A literature review

Resumo: | Chronic diseases (CD) comprise a group of chronic conditions that are generally associated with multiple causes, their onset is gradual and the prognosis is usually uncertain with a long or indefinite duration, which leads the patient to make chronic and often irregular use of the medication. This study aims to understand the pharmacotherapeutic profile of patients with CD and the main factors that influence poor adherence to pharmacological treatment. Data collection was based on studies published in the Scientific Electronic Library Online (SciELO), Pubmed and Academic Google databases using descriptors that most resembled our objectives. The main medications used in the cardiovascular system were angiotensin-converting enzyme inhibitors (ACEI), angiotensin I receptor blockers (ARB), thiazide diuretics and calcium channel blockers (BCC). Of the medications used in glycemic control, the most used were the biguanides followed by the sulfonureas, the prescriptions followed the main guidelines of the Brazilian Society of Cardiology (SBC) and Brazilian Society of Endocrinology and Metabolism (SBEM), however, the use of medications without prescription medica is still a reality that contributes to the appearance of complications and unwanted effects of medications. Finally, it was shown that the understanding of the pharmacotherapeutic profile as well as the factors that influence adherence need to be highlighted and worked on in the best way in order to contribute to therapeutic success and minimize the potential complications of poorly controlled chronic conditions. |

Este estudio tiene como objetivo conocer el perfil farmacoterapéutico de los pacientes con EC y los principales factores que influyen en la mala adherencia al tratamiento farmacológico. La recogida de datos se basó en estudios publicados en las bases de datos Scientific Electronic Library Online (SciELO), Pubmed y Academic Google utilizando descriptores que más se asemejaban a nuestros objetivos. Los principales medicamentos utilizados en el sistema cardiovascular fueron los inhibidores de la enzima convertidora de angiotensina (IECA), los bloqueadores de los receptores de angiotensina I (BRA), los diuréticos tiazídicos y los bloqueadores de los canales de calcio (BCC). De los medicamentos utilizados en el control glucémico, los más utilizados fueron las biguanidas seguidas de las sulfonureas, las prescripciones siguieron las principales directrices de la Sociedad Brasileña de Cardiología (SBC) y Sociedad Brasileña de Endocrinología y Metabolismo (SBEM), sin embargo, el uso de Los medicamentos sin receta médica siguen siendo una realidad que contribuye a la aparición de complicaciones y efectos no deseados de los medicamentos. Finalmente, se demostró que es necesario resaltar y trabajar de la mejor manera la comprensión del perfil farmacoterapéutico y los factores que influyen en la adherencia para contribuir al éxito terapéutico y minimizar las posibles complicaciones de las enfermedades crónicas mal controladas.As doenças crônicas (DC) compõem um grupo de condições crônicas que em geral são associadas a múltiplas causas, tem seu início gradual e o prognóstico usualmente incerto com longa ou indefinida duração, o que leva o paciente a fazer o uso crônico e muitas vezes irregular da medicação. O presente estudo tem como objetivo conhecer o perfil farmacoterapêutico dos pacientes com DC e os principais fatores que influenciam na má adesão ao tratamento farmacológico. A coleta de dados foi feita a partir de estudos publicados nas bases de dados Scientific Electronic Library Online (SciELO), Pubmed e Google Acadêmico utilizando descritores que mais se assemelhavam aos nossos objetivos. As principais medicações utilizadas no sistema cardiovascular foram as Inibidoras da enzima conversora de angiotensina (IECA), bloqueadores do receptor da angiotensina I (BRA), diuréticos tiazídicos e bloqueadores do canal de cálcio (BCC). Das medicações utilizadas no controle glicêmico, as mais utilizadas foram as biguanidas seguidas das sulfoniureias, as prescrições seguiram as principais orientações da Sociedade brasileira de cardiologia (SBC) e Sociedade brasileira de endocrinologia e metabologia (SBEM), entretanto, o uso de medicações sem prescrição medica ainda é uma realidade que contribui para o aparecimento de complicações e efeitos indesejados das medicações.
