Adverse events and nursing diagnosis in patients undergoing hematopoietic stem cell transplantation

Resumo: The aim of this study was to describe the adverse events presented by patients on the day of infusion of hematopoietic progenitor cells and to identify corresponding nursing diagnosis. Prospective cross-sectional study, whose sample consisted of 62 patients who underwent hematopoietic stem cell transplantation (allogeneic and autologous). After ethical approval, data collection took place over a period of 16 months in a public hospital in the state of São Paulo. About 53% of the transplant recipients were male, 54.8% had at least one comorbidity and 61.3% underwent allogeneic transplantation. For each type of adverse event (by organic system) presented, at least one nursing diagnosis was indicated, totaling 26 diagnosis and the most frequent ones were "risk of bleeding", "risk of allergic response", "fatigue" and "risk of impaired cardiovascular function". The highest frequency of elected nursing diagnosis belonged to the “security / protection” domain (n = 11), followed by the “activity / rest” domain (n = 5). This study found a higher frequency of nursing diagnosis belonging to the safety/protection domain and the activity/rest domain, which can be a reference for nurses in planning nursing care, with a view to the quality of care and the safety of onco-hematological patients.

Estudio prospectivo de corte transversal, cuya muestra consistió en 62 pacientes sometidos a trasplante de células madre hematopoyéticas, alogénicas y autólogas. Después de la aprobación ética, la recopilación de datos tuvo lugar durante un período de 16 meses en un hospital público en el estado de São Paulo. Alrededor del 53% de los receptores de trasplante eran hombres, el 54,8% tenía al menos una comorbilidad y el 61,3% se sometió a un trasplante alogénico. Para cada tipo de evento adverso (por sistema orgánico) presentado, se indicó al menos un diagnóstico de enfermería, con un total de 26 diagnósticos y los más frecuentes fueron "riesgo de sangrado", "riesgo de respuesta alérgica", "fatiga" y "riesgo de deterioro de la función cardiovascular". La frecuencia más alta de diagnósticos de enfermería elegidos pertenecía al dominio de "seguridad / protección" (n = 11), seguido del dominio de "actividad / descanso" (n= 5). Este estudio encontró una mayor frecuencia de diagnósticos de enfermería pertenecientes al dominio de seguridad/protección y el dominio de actividad/descanso, que puede ser una referencia para las enfermeras en la planificación de la atención de enfermería, con miras a la calidad de la atención y la seguridad del paciente onco -hematológico.O objetivo do estudo foi descrever os eventos adversos apresentados pelos pacientes no dia da infusão de células progenitoras hematopoiéticas e identificar os diagnósticos de enfermagem correspondentes. Estudo transversal prospectivo, cuja amostra foi composta por 62 pacientes submetidos ao transplante de células-tronco hematopoiéticas, alogênico e autólogo. Após aprovação ética, a coleta de dados ocorreu por um período de 16 meses em um hospital público do Estado de São Paulo. Cerca de 53% dos transplantados era do sexo masculino, 54,8% apresentava pelo menos uma comorbidade e 61,3% foi submetido ao transplante alogênico. Para cada tipo (por sistema orgânico) de evento adverso apresentado foi indicado, no mínimo, um diagnóstico de enfermagem, totalizando 26 diagnósticos e os mais frequentes foram “risco de sangramento”, “risco de resposta alérgica”, “fadiga” e “risco de função cardiovascular prejudicada”. A maior frequência de diagnósticos de enfermagem eleitos pertencia ao domínio “segurança/proteção” (n=11), seguido do domínio “atividade/repouso” (n=5).
