Dental implants and osteonecrosis of maxilaries associated with biphosphonate: an integrative review

Resumo: | The great advancement of implantology in the last 50 years has led to a great demand for oral rehabilitation, however, a great number of prescriptions of antireabsorption drugs, such as bisphosphonate, have been observed, which are related to adverse effects such as osteonecrosis. This integrative review aims to evaluate the adverse effects of oral rehabilitation with dental implants in patients undergoing therapy with bisphosphonate. We investigated studies that included the period from 2008 to 2018, in the following PubMed, Scienc Direct and Biblioteca virtual em saúde. A total of 193 articles on the subject were found, after the application of the inclusion and exclusion criteria and the removal of the duplicates, the review had 8 articles for analysis. With a total sample of 805 patients and 705 implants placed, we had a rate of osteonecrosis in 10.18% of the patients, of which 82.92% were females and a developmental preference was observed in the mandible. With these results we can observe that there is an adverse effect on the rehabilitation with dental implants in these patients, being necessary an individual evaluation for the prevention of cases of osteonecrosis. |

Esta revisión integradora tiene como objetivo evaluar los efectos adversos de la rehabilitación oral con implantes dentales en pacientes sometidos a terapia con bisfosfonatos. Investigamos estudios que incluyeron el período de 2008 a 2018, en los siguientes PubMed, Scienc Direct y Biblioteca virtual em saúde. Se encontraron un total de 193 artículos sobre el tema, luego de la aplicación de los criterios de inclusión y exclusión y la remoción de los duplicados, la revisión contó con 8 artículos para análisis. Con una muestra total de 805 pacientes y 705 implantes colocados, tuvimos una tasa de osteonecrosis en el 10,18% de los pacientes, de los cuales el 82,92% eran mujeres y se observó una preferencia de desarrollo en la mandíbula. Con estos resultados podemos observar que existe un efecto adverso sobre la rehabilitación con implantes dentales en estos pacientes, siendo necesaria una evaluación individual para la prevención de casos de osteonecrosis.O grande avanço da implantodontia nos últimos 50 anos, levou a uma grande procura para utilização na reabilitação oral, contudo vem sendo observado uma grande quantidade de prescrições de medicamentos antirreabsortivos, por exemplo o bifosfonato, que estão relacionados a efeitos adversos como a osteonecrose. Esta revisão integrativa tem o objetivo de avaliar os efeitos adversos da reabilitação oral com implantes dentários em pacientes submetidos a terapia com o bifosfonato. Foi pesquisado estudos que compreendiam o período de 2008 a 2018, nas seguintes bases de dados PubMed, Scienc Direct e Biblioteca virtual em saúde. Encontrados 193 artigos sobre o tema, após a aplicação dos critérios de inclusão e exclusão e retirada dos duplicados, a revisão contou com 8 artigos para análise. Com amostra total de 805 pacientes e 705 implantes colocados, obtivemos uma taxa de acometimento da osteonecrose em 10,18% dos pacientes, destes 82,92% do sexo feminino e observado uma predileção de desenvolvimento na mandíbula.
