Clinical and epidemiological investigation of the treatment of patients with inhibitory alloatibodies in hemophilia type A treated at a blood center in northeastern Brazil

Resumo: The present study aims to analyze the epidemiological profile, diagnosis and treatment of hemophiliac patients who developed inhibitors and who are treated by the Hemotherapy Center of Sergipe (HEMOSE). The research consists of a retrospective analysis based on information contained in the medical records of patients with hemophilia A who were treated in the outpatient sector of HEMOSE during the period from January 2010 to January 2018. For this purpose, variables such as gender were evaluated , age, location, color, severity, symptoms, inhibitor titer, and treatment. Of the patients evaluated, 17 (18%) had inhibitors, and, of this total, 11 (64.7%) were brown. In addition, most patients were diagnosed with hemophilia A and detected the presence of inhibitor in the age group between 0 and 10 years. Regarding the severity of the disease, 11 (64.7%) had the severe form. With regard to treatment before the development of inhibitors, the majority, 10 (59%), used an association of Factor VIII with Factor VIIIr. It is also noteworthy that the majority of patients developed low titer antibodies, with a rate of 82% (14). It is also considered that the results found in this study are in accordance with what is found in the literature on the subject and that the information obtained provides a better understanding of the profile of patients with hemophilia A who have inhibitors, which can then improve care. of these patients.

La investigación consiste en un análisis retrospectivo a partir de la información contenida en las historias clínicas de los pacientes con hemofilia A que fueron atendidos en el sector ambulatorio de HEMOSE durante el periodo comprendido de enero de 2010 a enero de 2018. Para ello se evaluaron variables como género, edad, ubicación, color, gravedad, síntomas, título de inhibidor y tratamiento. De los pacientes evaluados, 17 (18%) tenían inhibidores y, de ese total, 11 (64,7%) eran pardos. Además, la mayoría de los pacientes fueron diagnosticados con hemofilia A y se detectó la presencia de inhibidor en el grupo de edad entre 0 y 10 años. En cuanto a la gravedad de la enfermedad, 11 (64,7%) tenían la forma grave. En cuanto al tratamiento previo al desarrollo de inhibidores, la mayoría, 10 (59%), utilizaba una asociación de Factor VIII con Factor VIIIr. También es de destacar que la mayoría de los pacientes desarrollaron anticuerpos de bajo título, con una tasa del 82% (14). Dos pacientes avaliados, 17 (18%) apresentaram inibidores, e, desse total, 11 (64,7%) eram pardos. Além disso, a maioria dos pacientes foi diagnosticada com a hemofilia A e detectou a presença de inibidor na faixa etária entre 0 a 10 anos. Em relação à severidade da doença, 11 (64,7%) apresentaram a forma grave. Já no que diz respeito ao tratamento antes do desenvolvimento dos inibidores, a maioria, 10 (59%), utilizava uma associação do Fator VIII com o Fator VIIIr. Destaca-se ainda que os pacientes desenvolveram, em sua maioria, anticorpos de baixa titulação, com um índice de 82% (14).
