Functional demands and environmental factors: a geospatial analysis in users of primary health care

Resumo: Objectives: to evaluate and analyze the association between functional demands and environmental factors of users of the primary health care (PHC). Methods: cross-sectional study that evaluated PHC users using the Problem Survey for Rehabilitation Protocol (PLPR) and the Inventory of Environmental Factors (CHIEF). Descriptive statistics, geo-spatial analysis and Multiple Linear Regression were applied for data analysis. Results: 230 users were evaluated and presented greater functional demand in the pain domains (mean=5.29; SD=3.55); energy and sleep (mean=3.17; SD=2.91); and mobility (mean=3.07; SD=2.51). The environmental factors that presented the most problems were 'services and assistance' (mean=0.66; SD=0.61) and 'physical structure' (mean=0.61; SD=0.65). The geo-spatial analysis identified that the regions covered by the basic health unit with the greatest presence of barriers corresponded to places of residence of users with more functional needs. Worse health perception (p=0.000), greater number of diagnosed diseases (p=0.000) and greater presence of barriers in 'services and assistance' (p=0.039) and 'physical structure' (p=0.000) were predictors of greater functional demand. The adjusted coefficient of the model was 43.2% (p=0.001). Final considerations: Users of primary health care have mild to moderate difficulties or deficiencies like pain, mobility and energy and sleep, which can be identified already in the embracement to the service. The most present environmental barriers in the evaluated territory were in 'services and assistance' and 'physical structure'. Better health perception, lower number of diagnosed diseases and lower presence of environmental barriers are predictors of greater functionality in PHC users.

Métodos: estudio transversal que evaluó a los usuarios de la APS mediante el Protocolo de Encuesta de Problemas para la Rehabilitación (PLPR) y el Inventario de Factores Ambientales (CHIEF). Para el análisis de datos se aplicó estadística descriptiva, análisis geoespacial y Regresión Lineal Múltiple. Resultados: Se evaluaron 230 usuarios que presentaron mayor demanda funcional en los dominios del dolor (media=5,29; DE=3,55); energía y sueño (media=3,17; DE=2,91); y movilidad (media=3,07; DE=2,51). Los factores ambientales que presentaron más barreras fueron 'servicios y asistencia' (media=0,66; DE=0,61) y 'estructura física' (media=0,61; DE=0,65). El análisis geoespacial identifico quw las regiones del territorio de cobertura de la unidad básica de salud con mayor presencia de barreras correspondieron al lugar de residencia de los usuarios con más necesidades funcionales. Peor percepción de salud (p=0,000), mayor número de enfermedades diagnosticadas (p=0,000) y mayor presencia de barreras en 'servicios y asistencia' (p=0,039) y 'estructura física' (p=0,000) fueron predictores de mayor demanda funcional. El coeficiente ajustado del modelo fue de 43,2% (p=0,001). Consideraciones finales: Los usuarios de la atención primaria de salud presentan dificultades o deficiencias leves a moderadas en las áreas de dolor, movilidad y energía y sueño, las cuales pueden ser identificadas ya en la recepción al servicio. Las barreras ambientales más presentes en el territorio evaluado fueron en 'servicios y asistencia' y 'estructura física'. Mejor percepción de salud, menor número de enfermedades diagnosticadas y menor presencia de barreras ambientales son predictores de mayor funcionalidad en los usuarios de APS.Objetivos: avaliar e analisar a associação entre demandas funcionais e fatores ambientais de usuários atendidos na atenção primária em saúde (APS). Resultados: Foram avaliados 230 usuários que apresentaram maior demanda funcional nos domínios dor (média=5,29; DP=3,55); energia e sono (média=3,17; DP=2,91); e mobilidade (média=3,07; DP=2,51). Os fatores ambientais que apresentaram mais barreiras foram ‘serviços e assistência’ (média=0,66; DP=0,61) e ‘estrutura física’ (média=0,61; DP=0,65). A análise geo-espacial identificou as regiões do território de abrangência da unidade básica de saúde com maior presença de barreiras e estas foram correspondentes ao local de moradia dos usuários com mais necessidades funcionais. Pior percepção de saúde (p=0,000), maior número de doenças diagnosticadas (p=0,000) e maior presença de barreiras em ‘serviços e assistência’ (p=0,039) e ‘estrutura física’ (p=0,000) foram preditores de maior demanda funcional. O coeficiente ajustado do modelo foi de 43,2% (p=0,001). Considerações finais: Usuários da atenção primária em saúde apresentam dificuldades ou deficiências leves a moderadas nas áreas de dor, mobilidade e energia e sono, que podem ser identificadas já no acolhimento ao serviço. As barreiras ambientais mais presentes no território avaliado foram em ‘serviços e assistência’ e ‘estrutura física’.
