Impact of poverty on Drug Resistant Tuberculosis: A review

Resumo: Tuberculosis (TB) is a chronic infectious disease, caused by Mycobacterium tuberculosis, responsible for the most deaths yearly worldwide. One of the greatest obstacles in its approach, according to the World Health Organization (WHO), is the rising prevalence of drug-resistant strains, leading to treatment failure and higher mortality. The objective of this study was to carry out an integrative review of the medical literature in order to identify and analyze the impact of poverty on the incidence, prevalence and effectiveness of combating drug-resistant tuberculosis. (TBDR). A systematic literature review was carried out in Pubmed, Virtual Health Library (BVS) and Scielo databases with keywords “Drug-Resistant Tuberculosis” AND “Poverty”. Included in this review were articles published in the last 10 years in Portuguese, Spanish and English available as a free full text. There was great agreement in findings not only affirming the relationship between poverty and the occurrence of TBDR but also in the mechanisms by which it affects treatment compliance, among of which were: bigger costs to access medical care, loss of income due to work absence, prejudice, discrimination and poor food intake. Social determinants still play an unequivocal role in the incidence of TB and even more in its resistant forms, with great compromise of treatment efficacy and compliance. As a result, diminishing poverty and inequality should be considered pivotal strategies against this disease, even more so than the development of new diagnostic and treatment tools.

Uno de los mayores obstáculos para su lucha, según la Organización Mundial de la Salud (OMS), es la creciente prevalencia de cepas resistentes a la terapia estándar con medicamentos, que culmina en la refractariedad al tratamiento y una mayor mortalidad. El objetivo de este estudio fue realizar una revisión integradora de la literatura médica para identificar y analizar el impacto de la pobreza en la incidencia, prevalencia y efectividad en el combate a la tuberculosis resistente a drogas (DRTB). La Revisión Integrativa de la Literatura fue realizada en las bases de datos PubMED, Biblioteca Virtual en Salud (BVS) y Scielo, utilizando los descriptores “Tuberculosis resistente a los medicamentos” Y “Pobreza”. Se incluyeron artículos publicados en los últimos 10 años, en portugués, inglés y español, disponibles en su totalidad. Hubo gran concordancia entre los trabajos, no solo en afirmar categóricamente la relación entre la TBDR y la pobreza, sino también en los mecanismos por los cuales ésta afecta el curso del tratamiento, entre ellos: aumento de los gastos para acceder a la atención médica, pérdida de ingresos por ausencia del trabajo, prejuicio, discriminación e inseguridad alimentaria. Los determinantes sociales continúan jugando un papel inequívoco en la aparición de TB y más aún en sus formas resistentes, con pérdidas significativas en la efectividad y adherencia al tratamiento. Por tanto, la lucha contra la pobreza y las desigualdades se considera fundamental como estrategia primaria contra la enfermedad, incluso más que el desarrollo de nuevas tecnologías terapéuticas y diagnósticas.A tuberculose (TB) é uma doença infecciosa crônica, causada pelo Mycobacterium tuberculosis, sendo a que mais causa mortes no mundo. Um dos maiores obstáculos para seu combate, segundo a Organização Mundial da Saúde (OMS), é a crescente prevalência de cepas resistentes à terapia medicamentosa padrão, culminando em refratariedade ao tratamento e maior mortalidade. O objetivo deste estudo foi realizar uma revisão integrativa da literatura médica com o intuito de identificar e analisar o impacto da pobreza sobre a incidência, a prevalência e a eficácia do combate à Tuberculose drogarresistente (TBDR). A Revisão Integrativa de literatura foi realizada nas bases de dados PubMED, Biblioteca Virtual em Saúde (BVS) e Scielo, usando os descritores “Drug-Resistant Tuberculosis” AND “Poverty”. Foram incluídos artigos publicados nos últimos 10 anos, nos idiomas português, inglês e espanhol e disponíveis na íntegra. Houve grande concordância entre os trabalhos, não só ao afirmar categoricamente a relação entre TBDR e pobreza, mas também nos mecanismos pelos quais ela afeta o curso de tratamento, dentre eles: aumento de gastos para acessar o cuidado médico, perda de renda por afastamento laboral, preconceito, discriminação e insegurança alimentar. Os determinantes sociais continuam tendo papel inequívoco na ocorrência da TB e mais ainda em suas formas resistentes, com prejuízos significativos na eficácia e adesão ao tratamento.
