Alveolysis in primary teeth associated with dental trauma: Case report

Resumo: Alveolysis is characterized as the resorption of the alveolar bone with subsequent exposure of primary tooth root in the oral cavity. The objective of this study was to report a clinical case of a 6-year-old female patient, who attended the Primary Health Care in an inland city of Paraíba, with alveolysis after dental trauma. Clinical examination revealed the presence of alveolysis in the teeth 51 and 52 with lesion characterized by the mobility of dental elements and an extensive gingival vertical lesion in the vestibular region (51). The indicated treatment was extraction of both teeth and clinical follow-up for the others. The clinical conduct started with a psychological adaptation of the patient because she had suffered a trauma and still felt pain in the region. It is concluded that, in the case of a dental emergency, it is necessary to remove primary teeth with alveolysis, thus, avoiding injuries to subsequent permanent teeth and adjacent soft tissues. Pediatric dentistry needs more case reports on this subject so that the etiology and clinical conduct can follow the precepts of evidence-based dentistry.

Paciente de 6 años asistió a la Unidad Básica de Salud de una ciudad en el interior de Paraíba, con alveólisis después de un trauma dental. El examen clínico reveló la presencia de alveólisis en los elementos 51 y 52, una lesión que se caracterizó por la movilidad de los elementos dentales y una extensa lesión gingival en la región vestibular (51). El tratamiento indicado fue la extracción de los elementos mencionados y el seguimiento clínico de los demás. La conducta clínica comenzó con una adaptación psicológica de la paciente, ya que había sufrido un trauma y sintió dolor en la región. Se concluye que, en el caso de una emergencia dental, es necesario extraer los dientes primarios con alveólisis, evitando lesiones en los dientes permanentes y los tejidos blandos adyacentes. La odontología pediátrica necesita más informes sobre este tema para que la etiología y la conducta clínica puedan seguir los preceptos de la odontología basada en la evidencia.A alveólise é caracterizada como a reabsorção da tábua óssea alveolar com subsequente exposição da raiz do dente decíduo na cavidade bucal. Apresenta etiologia não esclarecida, apesar de frequentemente estar associada com infecções decorrentes de cárie e o traumatismo dentário. Objetivou-se neste estudo relatar um caso clínico de uma paciente de 6 anos de idade, sexo feminino, que compareceu a Unidade Básica de Saúde de uma cidade no interior da Paraíba, com alveólise após trauma dentário. Ao exame clínico observou-se a presença de alveólise nos elementos 51 e 52, lesão que foi marcada pela mobilidade dos elementos dentários e extensa lesão gengival no sentindo vertical, inciso-apical, na região vestibular (51). A raiz do elemento 51 apresentava-se exposta sem a proteção do osso alveolar e da mucosa e a do 52 com exposição da região apical. O tratamento indicado foi a exodontia dos elementos supracitados e acompanhamento clínico dos demais. A conduta clínica foi iniciada com uma adaptação psicológica da paciente, pois esta tivera sofrido um trauma e sentia dor na região. O protocolo clínico foi a anestesia tópica da região; anestesia terminal infiltrativa; sindesmotomia, luxação e exérese. Conclui-se que tratando-se de uma emergência odontológica é necessário a remoção de dentes decíduos com alveólise evitando-se lesões nos dentes permanentes subsequentes e tecidos moles adjacentes. O acompanhamento clínico dos demais elementos é de fundamental importância.
