Artifacts for the care of users with chronic skin lesions in primary health care based on principles of clinic management

Resumo: In this study, it is proposed to produce artifacts for the care of users with chronic skin lesions in primary health care (PHC). Research of an applied nature was undertaken, with a qualitative approach, with theoretical anchoring in principles of clinical management and systematized methodologically based on techniques and tools for experimenting with design. The analysis of the content produced integrates data from an integrative literature review and dialogues in focus groups, involving the multiprofessional PHC team. From this analysis, four thematic categories emerged, in which the respective artifacts produced are presented, with a view to qualifying care in PHC to users with chronic skin lesions: conceptual map, with graphic representation that allows an overview of the team's duties; care algorithm to support clinical decisions, care monitoring and evaluation; briefing that brings together the main therapeutic procedures with minimal resources and / or the option of special coverages fundamental to clinical and social management; and registration structure for the use of standards for monitoring, evaluation and decision support in care actions. The research contributes to the implementation of clinical management principles in PHC services, as it favors the incorporation of evidence-based practices, as well as the orientation of shared and interprofessional clinical actions, based on the needs of users with chronic injuries of skin, and quality and safety standards, focusing on undertaking best practices for the user when meeting the service.

Se realizó una investigación de carácter aplicado, con enfoque cualitativo, con anclaje teórico en principios de gestión clínica y sistematizada metodológicamente con base en técnicas y herramientas para experimentar con el diseño. El análisis del contenido producido integra datos de una revisión de literatura integradora y diálogos en grupos focales, involucrando al equipo multiprofesional de APS. De este análisis surgieron cuatro categorías temáticas, en las que se presentan los respectivos artefactos producidos, con miras a calificar la atención en la APS a los usuarios con lesiones cutáneas crónicas: mapa conceptual, con representación gráfica que permite una visión general de las funciones del equipo; algoritmo de atención para respaldar las decisiones clínicas, el seguimiento y la evaluación de la atención; briefing que reúne los principales procedimientos terapéuticos con mínimos recursos y / o la opción de coberturas especiales fundamentales para el manejo clínico y social; y estructura de registro para el uso de estándares de seguimiento, evaluación y apoyo a la decisión en las acciones de cuidado. La investigación contribuye a la implementación de los principios de gestión clínica en los servicios de APS, ya que favorece la incorporación de prácticas basadas en la evidencia, así como la orientación de acciones clínicas compartidas e interprofesionales, basadas en las necesidades de los usuarios con lesiones crónicas. de piel, y estándares de calidad y seguridad, enfocándose en emprender las mejores prácticas para el usuario en el cumplimiento del servicio.Neste estudo, propõe-se a produção de artefatos para o cuidado de usuários com lesões crônicas de pele na atenção primária à saúde (APS).
