Clinical and histological profile of gestational trophoblastic disease in the state of Piauí

Resumo: | This work aims to trace the clinical and histological profile of molar pregnancy in a tertiary maternity in the state of Piauí. This is a cross-sectional observational study with a quantitative approach. We analyzed 90 medical records of pregnant women with GTD between 14 and 50 years old admitted to a tertiary maternity hospital in Piauí between January 2016 and December 2019. Inclusion and exclusion criteria were used to select the records. A survey of variables was carried out, such as age, parity, beta HCG, clinical status, gestational age, method of emptying, histopathology, follow-up, contraceptive method and evolution. The results showed that only 0.6% of the women followed up during the study period developed GTD, the most expressive age group was between 20 and 30 years old. Regarding the histopathology, 73% were diagnosed with complete hydatidiform mole. Regarding the evolution of the disease, 73% of the patients progressed to cure, 7% developed GTN, 6% are in progress and 13% of them did not complete the follow-up. Therefore, the study was of great importance regarding the clinical and histological profile of women with GTD, since these factors are closely related to the evolution of the disease. The number of patients who did not complete the treatment is low, but it cannot be ignored, therefore, carrying out an active search for patients who did not complete the follow-up is essential. As limitations of this study, there is a probable loss of follow-up of some women, especially during the period of the covid-19 pandemic. |

Se trata de un estudio observacional transversal con enfoque cuantitativo. Se analizaron 90 prontuarios de gestantes con ETG entre 14 y 50 años internadas en una maternidad de tercer nivel en Piauí entre enero de 2016 y diciembre de 2019. Se utilizaron criterios de inclusión y exclusión para la selección de los prontuarios. Se realizó un levantamiento de variables como edad, paridad, beta HCG, estado clínico, edad gestacional, método de vaciado, histopatología, seguimiento, método anticonceptivo y evolución. Los resultados mostraron que solo el 0,6% de las mujeres seguidas durante el período de estudio desarrollaron ETG, el grupo de edad más expresivo fue el de 20 a 30 años. En cuanto a la histopatología, el 73% fueron diagnosticados de mola hidatiforme completa. En cuanto a la evolución de la enfermedad, el 73% de los pacientes progresó a la curación, el 7% desarrolló NTG, el 6% está en progreso y el 13% de ellos no completó el seguimiento. Por lo tanto, el estudio fue de gran importancia en cuanto al perfil clínico e histológico de las mujeres con ETG, ya que estos factores están íntimamente relacionados con la evolución de la enfermedad. El número de pacientes que no completaron el tratamiento es bajo, pero no se puede ignorar, por lo que es fundamental realizar una búsqueda activa de los pacientes que no completaron el seguimiento. Como limitaciones de este estudio, existe una probable pérdida de seguimiento de algunas mujeres, especialmente durante el período de la pandemia de covid-19.Esse trabalho tem como objetivo traçar o perfil clínico e histológico de gravidez molar em uma maternidade terciária do estado do Piauí. Trata-se de um estudo observacional do tipo transversal com abordagem quantitativa. Foram analisados 90 prontuários de gestantes portadoras de DTG entre 14 e 50 anos admitidas em uma maternidade terciária do Piauí entre janeiro de 2016 e dezembro de 2019. Para seleção dos prontuários foram utilizados critérios de inclusão e exclusão. Realizou-se o levantamento de variáveis, como idade, paridade, beta HCG, quadro clínico, idade gestacional, forma de esvaziamento, histopatológico, seguimento, método anticoncepcional e evolução. Os resultados evidenciaram que apenas 0,6% das mulheres acompanhadas no período do estudo desenvolveu DTG, a faixa etária de maior expressão foi de 20 a 30 anos. Em relação ao histopatológico, 73% foram diagnosticadas com mola hidatiforme completa. Em relação a evolução da doença, 73% das pacientes evoluíram para cura, 7% desenvolveram NTG, 6% estão em andamento e 13% delas não concluíram o seguimento. Portanto, o estudo apresentou grande importância quanto ao perfil clínico e histológico das mulheres com DTG, já que esses fatores estão intimamente relacionados à evolução da doença. A quantidade de pacientes que não concluiu o tratamento é baixa, porém não pode ser ignorada, logo, realizar a busca ativa das pacientes que não concluíram o seguimento é imprescindível.
