Epidemiological study of congenital heart disease in the State of Pará, Amazon, Brazil

Resumo: This study aimed to describe the clinical-epidemiological profile of cases of congenital heart disease treated at a referral hospital in the state of Pará - Brazil. Ecological, descriptive, retrospective, population-based study, where data from the register of patients treated between January 2012 and December 2016 were used, and the 2010 Census database was used to decode the information from the municipalities of origin. and the IBGE's millionth system. All patients had a diagnosis confirmed at least by echocardiogram. The variables were: gender, age, type of heart disease, treatment performed and waiting time. Of the 905 cases observed, 52% were over 1 year of age at diagnosis, 75.1% had acyanotic heart disease, with ventricular septal defect being the most frequent (21%). Among cyanotic heart diseases, Tetralogy of Fallot was the most frequent (12%). There was no gender predominance. At the time of evaluation, 55.7% of the patients were awaiting treatment, 33.8% had undergone surgery, 5.3% had undergone surgery and underwent hemodynamic treatment, and 5.2% had undergone treatment exclusively, hemodynamic. The lethality during the study period was 9.61%, of which 83.9% had undergone surgical treatment. There are difficulties in accessing diagnosis and treatment, especially in municipalities further away from the state capital.

Estudio ecológico, descriptivo, retrospectivo, de base poblacional, donde se utilizaron datos del registro de pacientes atendidos entre enero de 2012 y diciembre de 2016, y para la decodificación de las informaciones de los municipios de origen se utilizó la base de datos del Censo de 2010 y el millonésimo sistema del IBGE. Todos los pacientes tenían un diagnóstico confirmado al menos por ecocardiograma. Las variables fueron: sexo, edad, tipo de cardiopatía, tratamiento realizado y tiempo de espera. De los 905 casos observados, el 52% tenía más de 1 año de edad al diagnóstico, el 75,1% presentaba cardiopatía acianótica, siendo la comunicación interventricular la más frecuente (21%). Entre las cardiopatías cianóticas, la Tetralogía de Fallot fue la más frecuente (12%). No hubo predominio de género. Al momento de la evaluación, el 55,7 % de los pacientes estaban en espera de tratamiento, el 33,8 % había sido intervenido quirúrgicamente, el 5,3 % había sido intervenido quirúrgicamente y recibía tratamiento hemodinámico, y el 5,2 % había recibido tratamiento exclusivamente hemodinámico. La letalidad durante el período de estudio fue del 9,61%, de los cuales el 83,9% había sido sometido a tratamiento quirúrgico. Existen dificultades para acceder al diagnóstico y tratamiento, especialmente en los municipios más alejados de la capital del estado.Este estudo objetivou descrever o perfil clínico-epidemiológico dos casos de cardiopatia congênita atendidos em hospital de referência no estado do Pará – Brasil. Estudo ecológico, descritivo, retrospectivo, de base populacional, onde foram utilizados dados do cadastro dos pacientes atendidos no período entre janeiro de 2012 e dezembro de 2016, e para decodificação das informações dos municípios de procedência usou-se base de dados do Censo de 2010 e do sistema milionésimo do IBGE. Todos os pacientes tiveram diagnóstico confirmado pelo menos por ecocardiograma. As variáveis foram: gênero, idade, tipo de cardiopatia, tratamento realizado e tempo de espera. Dos 905 casos observados, 52% tinham mais de 01 ano de idade ao diagnóstico, 75,1% apresentaram cardiopatia acianótica, tendo sido a mais frequente a comunicação interventricular (21%). Entre as cardiopatias cianóticas, a Tetralogia de Fallot foi a mais frequente (12%). Não houve predomínio de gênero. No momento da avaliação, 55,7% dos pacientes encontravam-se aguardando tratamento, 33,8% haviam sido submetidos a cirurgia, 5,3% haviam sido operados e submetidos a tratamento hemodinâmico e 5,2% haviam sido submetidos exclusivamente a tratamento hemodinâmico. A letalidade durante o período estudado foi de 9,61%, dos quais 83,9% haviam sido submetidos a tratamento cirúrgico.
