Extrapyramidal effects and nutritional state associated with the use of antipsychotics in patients hospitalized in a psychiatric reference hospital, Belém-Pará

Resumo: | The schizophrenia is a serious/chronic mental disorder, characterized by disorders of thought, emotions and feelings, in which it compromises social interaction in various spheres. With the advent of psychopharmacotherapy in the 50 years of the last century, it represented a real revolution in the care of those who suffer from mental disorders. This study aimed to: evaluate the interference of the extrapyramidal effects caused by antipsychotics on the motor alteration and on the nutritional status of schizophrenic patients admitted to a psychiatric reference hospital. The research field was the Psychiatric Clinic of the State Public Foundation Hospital de Clínicas Gaspar Vianna, being performed with patients with schizophrenia, admitted to the Short Hospitalization Service, from August 2018 to February 2019. To assess the extrapyramidal effects, a pre-adapted form was used, based on the Simpson-Angus scale (Extrapyramidal effects scale) and the Barnes scale (Barnes akathisia scale). The anthropometric evaluation consisted of the body mass index, tricipital skinfold and waist circumference. For static analysis of data was performed the anova test and Pearson correlation test considering a p<0.05. The sample consisted of 81 individuals, 45.7% of patients used only first-generation antipsychotics, among which 62.2% manifested akathisia, while 40.5% had Parkinsonism. Regarding the nutritional status, 49.4% were in the eutrophic state. It was concluded that it is necessary to identify extrapyramidal symptoms in patients, knowing that these symptoms can worsen their health and increase their stay in the hospital. |

Con la psicofarmacoterapia, en los años 50, se produjo una auténtica revolución en la asistencia a los enfermos mentales. Este estudio tuvo como objetivo evaluar la interferencia de los efectos extrapiramidales causados por los antipsicóticos en la alteración motora y el estado nutricional de los pacientes esquizofrénicos ingresados en un hospital psiquiátrico de referencia. El ámbito de la investigación fue la Clínica Psiquiátrica de la Fundación Pública Estatal Hospital de Clínicas Gaspar Vianna, realizándose con pacientes con esquizofrenia, ingresados en el Servicio de Admisión Breve, en el periodo comprendido entre agosto de 2018 y febrero de 2019. Para la evaluación de los efectos extrapiramidales se utilizó un formulario preadaptado, basado en la escala de Simpson-Angus (escala de efectos extrapiramidales) y la escala de Barnes (escala de acatisia de Barnes). La evaluación antropométrica consistió en el índice de masa corporal, el grosor del pliegue cutáneo del tríceps y el perímetro de la cintura. Para el análisis estadístico se aplicó la prueba ANOVA y la correlación de Pearson, utilizando p<0,05. 81 pacientes participaron en el estudio, el 45,7% de los pacientes utilizaban sólo antipsicóticos de primera generación, entre ellos el 62,2% manifestaba acatisia, mientras que el 40,5% tenía parkinsonismo. En cuanto al estado nutricional, el 49,4% eran eutróficos. Se concluyó que la identificación de los síntomas extrapiramidales en los pacientes es necesaria, ya que estos síntomas pueden empeorar su salud y aumentar su estancia en el hospital.A esquizofrenia é um transtorno mental grave/crônico, caracterizada por desordens do pensamento, emoções e sentimentos, na qual compromete a interação social em seus diversos âmbitos. Com a psicofarmacoterapia, nos anos 50 do século passado, houve verdadeira revolução na assistência daqueles com transtornos mentais. Este estudo teve como objetivo: avaliar a interferência dos efeitos extrapiramidais provocados por antipsicóticos na alteração motora e no estado nutricional de pacientes esquizofrênicos admitidos em um hospital de referencia psiquiátrica. O campo da pesquisa foi a Clínica Psiquiátrica da Fundação Pública Estadual Hospital de Clínicas Gaspar Vianna, sendo realizada com pacientes portadores de esquizofrenia, admitidos no Serviço de Internação Breve, no período de agosto de 2018 a fevereiro de 2019. Para avaliação dos efeitos extrapiramidais utilizou-se um formulário pré-adaptado, com base na escala de Simpson-Angus (Escala de efeitos extrapiramidais) e na escala de Barnes (Escala de acatisia de Barnes). Avaliação antropométrica consistiu do índice de massa corporal, prega cutânea tricipital e circunferência da cintura. Para análise estatística foi aplicado o teste ANOVA e a correlação de Pearson, utilizando p<0.05. Participaram do estudo 81 pacientes. 45.7% faziam uso somente de antipsicóticos de primeira geração. Dentre esses 62.2% manifestaram acatisia, enquanto 40.5% apresentaram Parkinsonismo. Em relação ao estado nutricional 49.4% encontravam-se no estado de eutrofia.
