Use of leukocyte-platelet rich fibrin membrane in the treatment of gingival recession: a case report

Resumo: Leukocyte-platelet rich fibrin is an aggregate obtained by centrifuging the patient's own blood, it is rich in platelets, growth factors and cytokines and seems to improve the healing of the surgical wound and alleviate postoperative morbidity in cases of root coverage. The aim of this study is to report and discuss a clinical case of multiple gingival recessions treated with the association of the coronary repositioned flap and leukocyte-platelet rich fibrin. Patient, female, 28 years old, complaining of dentinal hypersensitivity and esthetic dissatisfaction. After physical and radiographic examinations, type 1 gingival recessions were observed in teeth 22, 23 and 24. Surgical planning included oblique paramarginal incisions starting from the cementoenamel junction of tooth 23 towards the gingival margin of adjacent teeth, flap detachment, then venipuncture and centrifugation at 2,494 revolutions per minute (700 g) for 12 minutes. At the end of this process, membranes were obtained, which were positioned and sutured over the gingival recessions, followed by coronary repositioning of the surgical papillae. In the postoperative period, satisfactory tissue repair was observed. Although the gold standard for root coverage is connective tissue grafting associated with the coronary repositioned flap, platelet aggregates stimulate the repair and regeneration of soft and hard tissues and can be a great option in some cases. In this clinical case, it was possible to perceive the beneficial effects of the use of leukocyte-platelet rich fibrin membrane in the treatment of gingival recessions.

El objetivo de este estudio es reportar y discutir un caso clínico de múltiples recesiones gingivales tratadas con la asociación del colgajo coronario reposicionado y fibrina rica en plaquetas y leucocitos. Paciente, mujer, 28 años, que se queja de hipersensibilidad dentinaria e insatisfacción estética. Después de los exámenes físicos y radiográficos, se observaron recesiones gingivales tipo 1 en los dientes 22, 23 y 24. La planificación quirúrgica incluyó incisiones paramarginales oblicuas a partir de la unión amelocementaria del diente 23 hacia el margen gingival de los dientes adyacentes, desprendimiento del colgajo, luego venopunción y centrifugación en 2494 revoluciones por minuto (700 g) durante 12 minutos. Al final de este proceso se obtuvieron las membranas, las cuales fueron posicionadas y suturadas sobre las recesiones gingivales, seguidas de reposicionamiento coronario de las papilas quirúrgicas. En el postoperatorio se observó reparación tisular satisfactoria. Aunque el estándar de oro para la cobertura radicular es el injerto de tejido conectivo asociado con el colgajo coronario reposicionado, los agregados de plaquetas estimulan la reparación y regeneración de tejidos blandos y duros y pueden ser una gran opción en algunos casos. En este caso clínico, fue posible percibir los efectos beneficiosos del uso de la membrana de fibrina rica en plaquetas y leucocitos en el tratamiento de las recesiones gingivales.A fibrina rica em plaquetas e leucócitos é um agregado obtido pela centrifugação do sangue do próprio paciente, é rica em plaquetas, fatores de crescimento e citocinas e parece melhorar a cicatrização da ferida cirúrgica e amenizar a morbidade pós-operatória em casos de recobrimento radicular. O objetivo deste trabalho é relatar e discutir um caso clínico de recessões gengivais múltiplas tratadas com a associação do retalho reposicionado coronário e a fibrina rica em plaquetas e leucócitos. Paciente, gênero feminino, 28 anos, com queixa de hipersensibilidade dentinária e insatisfação estética. Após exames físico e radiográfico, foram observadas recessões gengivais tipo 1 nos dentes 22, 23 e 24. O planejamento cirúrgico incluiu incisões oblíquas paramarginais partindo da junção cemento esmalte do dente 23 em direção à margem gengival dos dentes adjacentes, descolamento do retalho, em seguida a realização de punção venosa e centrifugação a 2.494 rotações por minuto (700 g) por 12 minutos. Ao final desse processo obteve-se as membranas, que foram posicionadas e suturadas sobre as recessões gengivais, seguido do reposicionamento coronário das papilas cirúrgicas. No período pós-operatório foi observado satisfatória reparação tecidual. Embora o padrão ouro para o recobrimento radicular seja o enxerto de tecido conjuntivo associado ao retalho reposicionado coronário, os agregados plaquetários estimulam a reparação e regeneração dos tecidos moles e duros e pode ser uma ótima opção em alguns casos.
