Bacteriological profile and variables related to neoplastic wound in patient under palliative care: cross-sectional study

Resumo: Objective: to verify the association between bacteriological profile and variables related to neoplastic wounds of patients under palliative care. Method: Cross-sectional study, with a sample composed of 36 patients admitted to the wards of the National Cancer Institute José Alencar Gomes da Silva - Unit IV. Data collection took place between the months of October and December 2018, using a specific form. In the data analysis, descriptive measures and Fischer/chi-square test were used at the 5% significance level (α = 0.05). Results: The sample was composed by 36 individuals, 19 (52.78%) of whom were female, with a predominant percentage of metastasis formation (63.89%). There is a strong association between the variables palliative radiotherapy and the presence of Pseudomonas sp. (p = 0.02), type of primary tumor and the presence of Citrobacter sp. and Acinetobacter sp. (p <0.001) and location of the neoplastic wound in the presence of Citrobacter sp. (p = 0.03) and Acinetobacter sp. (p = 0.01). Palliative chemotherapy does not favor an increase in biofilm in the neoplastic wound, and it is not associated with any bacterial genus. The main bacteria identified were Pseudomonas sp., Morganella sp., Klebsiella sp. and Proteus sp.. Conclusion: the treatment with radiotherapy, the type of primary tumor and the location of the tumor wound have a significant association with the bacteriological profile of the lesions and, therefore, the collection of swab and antibiogram analysis of the neoplastic wounds is recommended to improve nursing care .

Método: estudio transversal, con una muestra compuesta por 36 pacientes ingresados en las enfermarías del Instituto Nacional del Cáncer José Alencar Gomes da Silva - Unidad IV. La recopilación de datos ocurrió entre octubre y diciembre de 2018, utilizando un formulario específico. En el análisis de datos, se utilizaron medidas descriptivas y la prueba de Fischer/chi-cuadrado a un nivel de significancia del 5% (α = 0.05). Resultados: La muestra consistió en 36 individuos, 19 (52.78%) de los cuales eran mujeres, con un porcentaje predominante de formación de metástasis (63.89%). Existe una fuerte asociación entre las variables radioterapia paliativa y la presencia de Pseudomonas sp. (p = 0.02), tipo de tumor primario y la presencia de Citrobacter sp. y Acinetobacter sp. (p <0.001) y ubicación de la herida neoplásica en presencia de Citrobacter sp. (p = 0.03) y Acinetobacter sp. (p = 0.01). La falta de antibióticos no favorece un aumento de la biopelícula en la herida neoplásica. La quimioterapia paliativa no está asociada con ningún género bacteriano. Las principales bacterias identificadas fueron Pseudomonas sp., Morganella sp., Klebsiella sp. y Proteus sp.. Conclusión: el tratamiento con radioterapia, el tipo de tumor primario y la ubicación de la herida tumoral tienen una asociación significativa con el perfil bacteriológico de las lesiones y, por lo tanto, se recomienda la recolección de swab y análisis de antibiograma de las heridas neoplásicas para mejorar la atención de enfermería.Objetivo: verificar a associação entre perfil bacteriológico e variáveis relacionadas às feridas neoplásicas dos pacientes sob cuidados paliativos. A coleta de dados ocorreu entre os meses de outubro e dezembro de 2018, sendo utilizado um formulário próprio. Na análise dos dados utilizaram-se medidas descritivas e teste de Fischer/qui-quadrado ao nível de significância de 5% (α = 0,05). Resultados: A amostra foi composta por 36 indivíduos, sendo 19 (52,78%) do sexo feminino, com percentual predominante de formação de metástases (63,89%). Há forte associação entre as variáveis radioterapia paliativa e a presença de Pseudomonas sp. (p = 0,02), tipo de tumor primário e a presença de Citrobacter sp. e Acinetobacter sp. (p < 0,001) e localização da ferida neoplásica a presença de Citrobacter sp. (p = 0,03) e Acinetobacter sp. (p = 0,01). A não utilização de antibiótico não favorece o aumento do biofilme na ferida neoplásica. A quimioterapia paliativa não se associa a nenhum gênero bacteriano. As principais bactérias identificadas foram Pseudômonas sp., Morganella sp., Klebsiella sp. e Proteus sp..
