Mechanisms of hemodialysis and peritoneal dialysis

Resumo: Dialysis is a treatment that replaces some of the kidney functions, such as elimination of fluids, acid-base control, elimination of potassium and protein metabolism residues. Dialysis treatment is medicated with erythropoietin and vitamin supplementation. There are two types of dialysis: hemodialysis, where the patient's blood is filtered by a machine called a dialyzer, which is where ultrafiltration and blood diffusion occurs; and peritoneal dialysis that uses the peritoneum, a semipermeable membrane located in the abdomen that filters blood effectively. Renal failure that can lead to dialysis treatments can be identified from some laboratory tests such as: urea, creatinine, glomerular filtration rate, creatinine clearance, cystatin C, abnormal urinary sediment elements and renal biopsies. Aiming to understand the mechanisms of hemodialysis and peritoneal dialysis, through a research carried out by literature review, having as a means of theoretical foundation the most relevant and current scientific articles available online on the topic addressed. The mechanism of hemodialysis and peritoneal dialysis is basically by exchange of solutes by the concentration gradient, the waste left by the malfunction of the kidneys are drained from the blood or peritoneal space to the dialysate, a solution prepared to purify the blood/peritoneal fluid. Early diagnosis is extremely important, the kidneys are vital organs and are part of a complex that works to maintain the body's hemostasis.

Hay dos tipos de diálisis: la hemodiálisis, donde la sangre del paciente es filtrada por una máquina llamada dializador, que es donde se produce la ultrafiltración y difusión de la sangre; y diálisis peritoneal que utiliza el peritoneo, una membrana semipermeable ubicada en el abdomen que filtra la sangre de manera efectiva. La insuficiencia renal que puede conducir a tratamientos de diálisis se puede identificar a partir de algunos exámenes de laboratorio como: urea, creatinina, tasa de filtración glomerular, depuración de creatinina, cistatina C, elementos anormales del sedimento urinario y biopsias renales. Con el objetivo de comprender los mecanismos de hemodiálisis y diálisis peritoneal, a través de una investigación realizada por revisión de literatura, teniendo como medio de fundamentación teórica los artículos científicos más relevantes y actuales disponibles en línea sobre el tema abordado. El mecanismo de la hemodiálisis y diálisis peritoneal es básicamente por intercambio de solutos por el gradiente de concentración, los desechos que deja el mal funcionamiento de los riñones son drenados de la sangre o espacio peritoneal al dializado, solución preparada para depurar la sangre/líquido peritoneal. El diagnóstico precoz es sumamente importante, los riñones son órganos vitales que forman parte de un complejo que trabaja para mantener la hemostasia del organismo.A diálise é um tratamento que substitui algumas das funções renais, como eliminação de líquidos, controle ácido-base, eliminação de potássio e restos do metabolismo proteico, não sendo possível a substituição da produção de eritropoetina e ativação da vitamina D, assim o paciente no tratamento de diálise é medicado com eritropoetina e suplementação de vitaminas. São dois tipos de diálise: a hemodiálise, onde o sangue do paciente é filtrado por uma máquina denominada dialisador, que é onde ocorre a ultrafiltração e difusão do sangue; e a diálise peritoneal que utiliza o peritônio, uma membrana semipermeável localizada no abdômen que filtra o sangue de maneira eficaz. A insuficiência renal que pode levar aos tratamentos de diálise pode ser identificada a partir de alguns exames laboratoriais como: ureia, creatinina, taxa de filtração glomerular, clearance de creatinina, cistatina C, elementos anormais do sedimento urinário e biópsias renais. Tendo como objetivo compreender os mecanismos da hemodiálise e diálise peritoneal, através de uma pesquisa realizada por revisão bibliográfica, tendo como meios de fundamentação teórica os artigos científicos disponíveis online mais relevantes e atuais sobre o tema abordado. O mecanismo da hemodiálise e diálise peritoneal se dá basicamente por troca de solutos pelo gradiente de concentração, os resíduos deixados pelo mal funcionamento dos rins são drenados do sangue ou espaço peritoneal para o dialisado, uma solução preparada para purificar o sangue/fluido peritoneal.
