Factors associated with early weaning: integrative review

Resumo: Exclusive breastfeeding is the best food for the newborn; and, when extended to 6 months, it is associated with adequate nutritional status, reduced infections, and lower mortality. Despite the nutritional and immunological superiority of breast milk, there are high rates of weaning due to several factors such as difficulties related to breastfeeding and lack of knowledge of the mother about the benefits of breast milk, among others. In this context, this study aimed to evaluate the scientific evidence about the factors associated with early weaning. This was an integrative literature review of articles published from 2016 to 2021 available in the BVS, PubMed and SciELO databases. Twelve studies were included, of which 25% were carried out in Brazil. The prevalence of early weaning ranged from 7% to 85.7%, with lower prevalences observed in Brazil. Main factors associated with early weaning were age and low educational level of the mother, cesarean delivery, use of pacifiers, return to work and maternal perception of low amount of milk, showing that both sociodemographic and obstetric factors as well as inherent to the mother's knowledge directly influence the duration of exclusive breastfeeding. However, it is necessary to investigate behavioral variables and serious psychiatric illnesses to understand the real determinants of early weaning, enabling health professionals to develop actions focusing mainly on health education.

A pesar de los problemas relacionados con la madre materna, existe un desconocimiento de la leche materna sobre los diversos beneficios maternos. En este objetivo evaluar los estudios asociados, los estudios sobre los factores de contexto asociados con el estudio temprano. Esta fue una revisión integrativa de la literatura de artículos publicados entre 2016 y 2021 disponibles en las bases de datos BVS, PubMed y SciELO. Se incluyeron 12 estudios, de los cuales 25% fueron realizados en Brasil. La prevalencia del destete varía del 7% al 85%, observándose una prevalencia temprana en Brasil. Los principales factores asociados al destete fueron la edad de la madre y bajo nivel educativo, parto por cesárea, uso de chupete, retorno a la percepción materna y percepción de la baja cantidad de leche, evidenciando que los factores sociodemográficos y obstétricos tempranos como factores inherentes al conocimiento de la madre madre lactancia materna directamente en la duración del tratamiento exclusivo. Sin embargo, se necesitan variables conductuales y psiquiátricas para comprender los verdaderos determinantes de la salud temprana, centrándose principalmente en la educación para la salud.O aleitamento materno exclusivo é o melhor alimento para o recém-nascido no período neonatal; e, quando estendido para 6 meses, está associado a estado nutricional adequado, redução de infecções e menor mortalidade. Apesar da superioridade nutricional e imunológica do leite materno, há altas taxas de desmame precoce devido a diversos fatores como dificuldades relacionadas à amamentação e falta de conhecimento da mãe sobre os benefícios do leite materno, entre outros. Nesse contexto, este estudo objetivou avaliar as evidências científicas acerca dos fatores associados ao desmame precoce. Tratou-se de uma revisão integrativa da literatura de artigos publicados no período de 2016 a 2021 disponíveis nas bases de dados BVS, PubMed e SciELO. Foram incluídos 12 estudos, dos quais, 25% foram realizados no Brasil. A prevalência de desmame precoce variou de 7% a 85,7%, com menores prevalências observadas no Brasil. Os principais fatores associados ao desmame precoce foram idade e baixo nível educacional da mãe, parto cesáreo, uso da chupeta, retorno ao trabalho e percepção materna de baixa quantidade de leite, evidenciando que tanto os fatores sociodemográficos e obstétricos quanto os fatores inerentes aos conhecimentos da mãe influenciam diretamente na duração do aleitamento materno exclusivo.
