The Expanded Clinic of the Dentistry course at UEM: a critical analysis based on the evaluation of records

Resumo: The objective was to identify the contributions and challenges linked to the dynamics of an expanded clinic in the context of undergraduate teaching in Dentistry. 381 medical records were evaluated (2012-2016) and data were extracted regarding the demographic profile, completion of fields, main complaint, treatment plan options, number of discharges and referrals. There was a higher prevalence of adult, Caucasian and female patients. The professors' signature fields and treatment plan options were satisfactorily completed. There was a higher frequency of unsatisfactory completion of the medical record in cases in which treatment began on the same day of admission. The referral rate has increased over the years by 58%. Discharge rates were higher for patients who had three options for treatment plans and who spent less time in the OR. In 2015, the number of discharges and referrals was higher compared to other years, as well as the number of treatment plan options. In the years 2012 and 2015, the completion of CA records reached its best rates. It can be concluded that the Expanded Clinic, as a space for reception, planning, execution and management of multidisciplinary and multiprofessional treatments, requires periodic evaluations and reflections that support future actions and the application of pedagogical resources, involving all participants and segments, since that it is a process in constant evolution and improvement.

Se evaluaron 381 historias clínicas (2012-2016) y se extrajeron datos sobre perfil demográfico, cumplimentación de campos, principal motivo de consulta, opciones de planes de tratamiento, número de altas y derivaciones. Hubo una mayor prevalencia de pacientes adultos, caucásicos y mujeres. Los campos de firma de los profesores y las opciones del plan de tratamiento fueron completados satisfactoriamente. Hubo una mayor frecuencia de cumplimentación insatisfactoria de la historia clínica en los casos en los que se inició el tratamiento el mismo día del ingreso. La tasa de referencia ha aumentado a lo largo de los años en un 58%. Las tasas de alta fueron más altas para los pacientes que tenían tres opciones de planes de tratamiento y que pasaban menos tiempo en el quirófano. En 2015, el número de altas y derivaciones fue mayor en comparación con otros años, así como el número de opciones de planes de tratamiento. En los años 2012 y 2015, la finalización de récords de CA alcanzó sus mejores índices. Se puede concluir que la Clínica Ampliada, como espacio de recepción, planificación, ejecución y gestión de tratamientos multidisciplinarios y multiprofesionales, requiere evaluaciones y reflexiones periódicas que apoyen acciones futuras y la aplicación de recursos pedagógicos, involucrando a todos los participantes y segmentos, ya que es un proceso en constante evolución y mejora.O objetivo foi identificar as contribuições e desafios atrelados à dinâmica de uma clínica ampliada no contexto do ensino de graduação em Odontologia. Para isso, foram avaliados 381 prontuários (2012-2016) e extraídos dados relativos ao perfil demográfico, preenchimento dos campos, queixa principal, opções de planos de tratamento, número de altas e encaminhamentos. Houve uma prevalência maior de pacientes adultos, leucodermas e do gênero feminino. Estavam satisfatoriamente preenchidos os campos de assinatura dos docentes e opções de plano de tratamento. Verificou-se maior frequência de preenchimento insatisfatório do prontuário nos casos em que o tratamento se iniciava no mesmo dia do acolhimento. A taxa de encaminhamento aumentou ao longo dos anos em 58%. Os índices de alta foram maiores para pacientes que apresentavam três opções de planos de tratamento e que permaneciam menos tempo na CA. No ano de 2015, o número de altas e encaminhamentos foi maior em comparação aos outros anos, bem como o número de opções de planos de tratamentos. Nos anos de 2012 e 2015, o preenchimento dos prontuários da CA atingiu seus melhores índices.
