Impact of caregiver guidance on infection prevention in home care

Resumo: Home care is an alternative to hospital admissions. Despite the multiple benefits, concerns about the risk of infections persist. Thus, the aim of this study was to assess the impact of supervised guidance on the care of patients hospitalized at home on the prevention of infections, using and comparing the conventional and systematic strategies. This is an experimental study with home care patients at Home Care service of a Medical Cooperative from Sergipe between May 2006 and April 2007. The patients included were divided into two groups, the control (group I) and the experimental (group II). Caregivers in group I received general guidelines, without standardization, while those in group II received guidelines systematized by the researcher. At the end, the occurrence of infections between the two groups was analyzed by an infectologist who was not aware of the allocation of patients in the groups. Forty-seven patients in group I, conventional, and 28 in group II, experimental, were evaluated. Infections were observed throughout the period of data collection in both groups and were diagnosed in 48.9% in group I and 17.9% in group II, with a significant difference between them and a relative risk of 2.7 for the occurrence of infection. Thus, this study showed a reduction in the occurrence of infections in the group that received systematic guidance in relation to the group that received general guidance, noting the effectiveness of this strategy.

A pesar de los múltiples beneficios, persisten las preocupaciones sobre el riesgo de infecciones. Así, el objetivo de este estudio fue evaluar el impacto de la orientación supervisada en la atención de los pacientes hospitalizados en el domicilio en la prevención de infecciones, utilizando y comparando las estrategias convencionales y sistemáticas. Se trata de un estudio experimental con pacientes domiciliarios del Servicio de Asistencia Domiciliaria de una Cooperativa Médica de Sergipe entre mayo de 2006 y abril de 2007. Los pacientes incluidos se dividieron en dos grupos, el control (grupo I) y el experimental (grupo II). Los cuidadores del grupo I recibieron pautas generales, sin estandarización, mientras que los del grupo II recibieron pautas sistematizadas por el investigador. Al final, la ocurrencia de infecciones entre los dos grupos fue analizada por un infectólogo que no conocía la asignación de pacientes en los grupos. Se evaluaron 47 pacientes del grupo I convencional y 28 del grupo II experimental. Las infecciones se observaron durante todo el período de recolección de datos en ambos grupos y se diagnosticaron en un 48,9% en el grupo I y en un 17,9% en el grupo II, con una diferencia significativa entre ellas y un riesgo relativo de 2,7 para la ocurrencia de infección. Así, este estudio mostró una reducción en la ocurrencia de infecciones en el grupo que recibió orientación sistemática en relación al grupo que recibió orientación general, destacando la efectividad de esta estrategia.A assistência domiciliar é uma alternativa às internações hospitalares. Assim, o objetivo deste estudo foi avaliar o impacto da orientação supervisionada aos cuidados dos pacientes internados no domicílio na prevenção de infecções, utilizando e comparando a estratégia convencional e a sistemática. Trata-se de um estudo experimental com pacientes em assistência domiciliar do serviço de Home Care de Cooperativa Médica em Sergipe entre maio de 2006 e abril de 2007. Os pacientes incluídos foram divididos em dois grupos, o controle (grupo I) e o experimental (grupo II). Os cuidadores do grupo I recebiam as orientações gerais, sem normatização, enquanto os do grupo II receberiam orientações sistematizadas pela pesquisadora. Ao final, foi analisada a ocorrência de infecções entre os dois grupos por infectologista que não tinha conhecimento acerca da alocação dos pacientes nos grupos. Foram avaliados 47 pacientes no grupo I, convencional, e 28 no grupo II, experimental. Foram observadas infecções durante todo o período da coleta de dados nos dois grupos e foram diagnosticadas em 48,9% no grupo I e 17,9% no grupo II, com diferença significante entre eles e risco relativo de 2,7 para ocorrência de infecção.
