Adrenal insufficiency: from concept to treatment

Resumo: Adrenal Insufficiency (AI) is a clinical condition resulting from the deficient synthesis of the hormone cortisol by the cortex of the glands located just above the kidneys. Cortisol performs several essential functions in the body, interfering with metabolism, blood pressure and inflammatory processes. AI can be classified as primary or secondary. The primary form occurs early on in the adrenal gland itself. Glandular inflammation, inherited enzyme deficits and infectious processes are the most common causes. The secondary form in this situation, the initial problem is in the hypothalamus or the pituitary gland, located at the base of the brain. Tumors in this portion or abrupt discontinuation of glucocorticoid therapy are the most frequently seen. The following article aimed to describe from concept to treatment of adrenal insufficiency. This is a narrative review of literature based on SciELO, MEDLINE, BVS, SBEM platforms, in the time frame of the last 5 years. There is a lot of information on the subject in the scientific literature. Possibly this underscores the importance of the topic, in which current data on the definition, classification, clinical picture, diagnosis and treatment of AI are made available in full. In view of the information collected, it can be clarified that AI is a syndrome with clinical implications that urge an early and differential diagnosis, investigation of the etiological cause and appropriate treatment according to the clinical condition of the patient and the form of presentation of AI.

El cortisol realiza varias funciones esenciales en el cuerpo, interfiriendo con el metabolismo, la presión arterial y los procesos inflamatorios. La IA se puede clasificar en primaria o secundaria. La forma primaria ocurre temprano en la propia glándula suprarrenal. La inflamación glandular, los déficits enzimáticos hereditarios y los procesos infecciosos son las causas más comunes. La forma secundaria en esta situación, el problema inicial está en el hipotálamo o la glándula pituitaria, ubicada en la base del cerebro. Los tumores en esta porción o la interrupción abrupta de la terapia con glucocorticoides son los que se observan con mayor frecuencia. El siguiente artículo tuvo como objetivo describir desde el concepto hasta el tratamiento de la Insuficiencia suprarrenal. Esta es una revisión narrativa de la literatura basada en las plataformas SciELO, MEDLINE, BVS, SBEM, en el marco temporal de los últimos 5 años. Hay mucha información sobre el tema en la literatura científica. Posiblemente esto resalte la importancia del tema, en el que se dispone de forma completa de los datos actuales sobre la definición, clasificación, cuadro clínico, diagnóstico y tratamiento de la IA. A la vista de la información recabada, se puede aclarar que la IA es un síndrome con implicaciones clínicas que urgen un diagnóstico precoz y diferencial, investigación de la causa etiológica y tratamiento adecuado según el cuadro clínico del paciente y la forma de presentación de la IA.A Insuficiência Adrenal (IA) é uma condição clínica resultante da síntese deficiente do hormônio cortisol pelo córtex das glândulas ademais, localizadas logo acima dos rins. O cortisol desempenha várias funções essenciais no organismo, interferindo no metabolismo, pressão arterial e processos inflamatórios. A IA pode ser classificada em primária ou secundária. A forma primária ocorre desde o início na própria glândula adrenal. Inflamações glandulares, déficits enzimáticos herdados e processos infecciosos são as causas mais comuns. A forma secundária nesta situação, o problema inicial é no hipotálamo ou na glândula hipófise, localizados na base do cérebro. Tumores nesta porção ou a eliminação abrupta da terapêutica com glicocorticoides são os mais vistos. O seguinte artigo objetivou descrever do conceito ao tratamento sobre a insuficiência adrenal. Trata-se de uma revisão narrativa de literatura fundamentada nas plataformas do SciELO, MEDLINE, BVS, SBEM, no recorte temporal dos últimos 5 anos. Na literatura científica existem muitas informações sobre a temática. Possivelmente isso ressalta a importância do tema, a qual são disponibilizados na íntegra dados atuais sobre definição, classificação, quadro clínico, diagnóstico e tratamento da IA.
