Resistance transmitted to antiretrovirals in pregnant women and newborns due to HIV-1: an integrative review

Resumo: HIV is one of the most important diseases in the world, with vertical transmission being one of the most combated problems, especially after the introduction of antiretroviral therapy. However, mutations in HIV due to transmitted drug resistance (TDRM) can affect the effectiveness of the first line of empirical antiretroviral therapy (ART) schemes and its prevalence is believed to be increasing in the pediatric population and in pregnant women. Thus, the objective was to investigate, through an integrative review, the resistance transmitted to antiretrovirals in pregnant women and newborns by HIV-1, focusing on mutations and associated factors, through a literature review of the last 10 years, using the LILACS, MEDLINE and SCIELO databases, which make up the Virtual Health Library. As a result, there were 566 articles, of which 71 were pre-selected to be read in full, resulting in 14 approved articles. It was observed that many of the accepted articles were studied in Brazil, the United States and South Africa. The presence of transmitted resistance negatively affects the clinical efficacy of antiretroviral prophylaxis during future pregnancies, leading to a worsening of the infection and disease progression to child, when there is infection by this virus. Therefore, future research and public health policies should be directed to the monitoring of TDRM trends in prevalence and relationship with clinically relevant results in pregnant women and newborns with HIV-I.

Sin embargo, las mutaciones del VIH debido a la resistencia a los medicamentos transmitidos (TDRM) pueden afectar la efectividad de la primera línea de esquemas empíricos de terapia antirretroviral (TAR) y se cree que su prevalencia está aumentando en la población pediátrica y en mujeres embarazadas. Por lo tanto, el objetivo era investigar, a través de una revisión integradora, la resistencia transmitida a los antirretrovirales en mujeres embarazadas y recién nacidos por el VIH-1, enfocándose en mutaciones y factores asociados, a través de una revisión de la literatura de los últimos 10 años, utilizando el Las bases de datos LILACS, MEDLINE y SCIELO, que conforman la Biblioteca Virtual en Salud. Como resultado, hubo 566 artículos, 71 de los cuales fueron preseleccionados para ser leídos en su totalidad, lo que resultó en 14 artículos aprobados. Desse modo, objetivou-se investigar, por meio de uma revisão integrativa a resistência transmitida aos antirretrovirais em gestantes e recém-nascidos pelo HIV-1, focando nas mutações e fatores associados, através de uma revisão da literatura dos últimos 10 anos, utilizando as bases de dados LILACS, MEDLINE e SCIELO, que compõe a Biblioteca Virtual em Saúde. Houve como resultado 566 artigos, dos quais 71 foram pré-selecionados para serem lidos na íntegra, resultando em 14 artigos aprovados. Observou-se que muitos dos artigos aceitos tinham como estudo no Brasil, Estados Unidos e África do Sul. A presença de resistência transmitida interfere negativamente na eficácia clínica da profilaxia antirretroviral durante as gestações futuras, levando a uma piora da infecção e progressão da doença a criança, com quando há infecção por esse vírus.
