Identification of Lophomonas spp in bronchoalveolar lavage of an individual with SARS by SARS-CoV-2 associated with pulmonary tuberculosis

Resumo: Infection by the protozoan Lophomonas spp is a rare condition, with just over 250 cases described worldwide, none of them in Brazil. This infection is mostly associated with states of immunosuppression and poor hygiene conditions in underdeveloped countries. The objective of this article is to report the first Brazilian case of bronchopulmonary lophomoniasis in a male individual concomitantly with Severe Acute Respiratory Syndrome (SRAG) caused by the new coronavirus (SARS-CoV-2) and pulmonary tuberculosis. During the investigation of the respiratory condition, the patient presented a solitary cavitated nodule in the imaging exam, being submitted to a bronchoalveolar lavage that showed the presence of the protozoan of the genus Lophomonas spp. Drug treatment with Metronidazole was started for 14 days, with clinical improvement, with interruption of fever and subsequent extubation. Thus, it is important to consider the variety of etiological possibilities in infectious conditions of the respiratory tract that do not respond to conventional therapy used for bacterial and fungal pneumonia.

Esta infección se asocia principalmente a estados de inmunosupresión y malas condiciones de higiene en países subdesarrollados. El objetivo de este artículo es reportar el primer caso brasileño de lofomoniasis broncopulmonar en un individuo masculino concomitantemente con Síndrome Respiratorio Agudo Severo (SRAS) causado por el nuevo coronavirus (SARS-CoV-2) y tuberculosis pulmonar. Durante la investigación de la afección respiratoria, la paciente presentó un nódulo solitario cavitado en el examen de imagen, siendo sometida a un lavado broncoalveolar que mostró la presencia del protozoo del género Lophomonas spp. Se inició tratamiento farmacológico con metronidazol durante 14 días, con mejoría clínica, con interrupción de la fiebre y posterior extubación. Por lo tanto, es importante considerar la variedad de posibilidades etiológicas en condiciones infecciosas del tracto respiratorio que no responden a la terapia convencional utilizada para la neumonía bacteriana y fúngica.A infecção pelo protozoário Lophomonas spp é uma condição rara, com pouco mais de 250 casos descritos no mundo, nenhum deles no Brasil. Esta infecção está associada em sua grande maioria a estados de imunossupressão e a condições de higiene precárias em países subdesenvolvidos. O objetivo desde artigo é relatar o primeiro caso brasileiro de lofomoníase broncopulmonar em indivíduo do sexo masculino concomitantemente a Síndrome Respiratória Aguda Grave (SRAG) pelo novo coronavírus (SARS-CoV-2) e tuberculose pulmonar. Durante a investigação do quadro respiratório, paciente apresentou nódulo solitário cavitado em exame de imagem, sendo submetido a lavado broncoalveolar que evidenciou a presença do protozoário do gênero Lophomonas spp. Foi iniciado tratamento medicamentoso com Metronidazol durante 14 dias, havendo melhora clínica, com interrupção da febre e posterior extubação.
