Neonatal mortality and risk factors in the state of Paraná: temporal trend from 2000 to 2016

Resumo: To analyze the series of neonatal mortality rates in the state of Paraná and risk factors between 2000 and 2016. This is an ecological-descriptive-analytical study using Quasi-Poisson and Gaussian regression models. The factors gender and age of the child were considered; age and education of the mother. Neonatal and early neonatal mortality rates were, on average, higher for boys at approximately 2.6 and 2.4 deaths per 1,000 live births, respectively, reduction of one death every eight years for girls and one death every four years for boys at both rates. Every five years the early neonatal mortality rate decreased by 16% for mothers up to 19 years old and 12% for mothers over 19. For neonatal mortality rate there was a decrease of 11% every five years in both age groups. Among mothers with up to seven years of education, there is a drop of 6% in the neonatal mortality rate and 11% at the beginning, every five years. In all cases the late neonatal mortality rate was not significant. There was a significant reduction in the neonatal mortality rate in the state of Paraná in any period evaluated, with early neonatal mortality emerging as the main component of the decay; late neonatal mortality remained constant. On average, the risk of neonatal death is higher for boys; for babies whose mothers are up to nineteen and among newborns of mothers with up to seven years of schooling.

Se trata de un estudio ecológico-descriptivo-analítico utilizando modelos de regresión Quasi-Poisson y Gaussiana. Los factores considerados fueron el sexo y la edad del niño; la edad y la educación de la madre. Las tasas de mortalidad neonatal y neonatal temprana fueron, en promedio, más altas para los niños, con aproximadamente 2,6 y 2,4 muertes por cada 1.000 nacidos vivos, respectivamente, hubo una reducción de una muerte cada ocho años para niñas y una muerte cada cuatro años para niños en ambas tasas. Cada cinco años, la tasa de mortalidad neonatal temprana disminuyó un 16% para las madres hasta los 19 años y un 12% para las madres mayores de 19. Para la tasa de mortalidad neonatal, hubo una disminución del 11% cada cinco años en ambos grupos de edad. Entre las madres con hasta siete años de escolaridad, hubo una caída del 6% en la tasa de mortalidad neonatal y del 11% en la tasa de mortalidad temprana, cada cinco años. En todos los casos la tasa de mortalidad neonatal tardía no fue significativa. Hubo una caída significativa en la tasa de mortalidad neonatal en el estado de Paraná en el período evaluado, emergiendo la mortalidad neonatal precoz como el principal componente de la caída; la mortalidad neonatal tardía se mantuvo constante. En promedio, el riesgo de muerte neonatal es mayor para los niños; para los bebés cuyas madres tengan hasta diecinueve años y entre los recién nacidos de madres con hasta siete años de escolaridad.Analisar as séries das taxas de mortalidade neonatal no estado do Paraná e fatores de risco entre os anos de 2000 a 2016. Trata-se de um estudo ecológico-descritivo-analítico utilizando modelos de regressão Quasi-Poisson e Gaussiano. Foram considerados os fatores sexo e idade da criança; idade e escolaridade da mãe. As taxas de mortalidade neonatal e neonatal precoce foram, em média, maiores para meninos em aproximadamente 2,6 e 2,4 mortes a cada 1.000 nascidos vivos, respectivamente, houve redução de uma morte a cada oito anos para meninas e uma morte a cada quatro anos para meninos em ambas as taxas. A cada cinco anos a taxa de mortalidade neonatal precoce decresceu 16% para mães até 19 anos e 12% para mães com mais de 19. Para a taxa de mortalidade neonatal houve queda de 11% a cada cinco anos nas duas faixas etárias. Entre mães com têm até sete anos de estudo houve queda de 6% na taxa de mortalidade neonatal e 11% na precoce, a cada cinco anos. Em todos os casos a taxa de mortalidade neonatal tardia não foi significativa. Houve queda significativa na taxa de mortalidade neonatal no estado do Paraná no período avaliado, com a mortalidade neonatal precoce despontando como o principal componente no decaimento; a mortalidade neonatal tardia manteve-se constante.
