Effect of non-surgical periodontal treatment on C-reactive protein, glycated hemoglobin and hyperlipidemia indexes in cardiac patients: a literature review

Resumo: | The present study analyzed the scientific literature in search of information that can contribute to the knowledge about the analysis of indexes that are considered factors of predisposition to heart problems after non-surgical periodontal treatment, more specifically lipid profile, glycated hemoglobin ( HbA1c) and C-reactive protein (PCR). As the present study is a literature review, for its production, the databases were consulted: Biblioteca Virtual em Saúde (BVS), Publicações Médicas (PubMed), Medline e biblioteca virtual Scientific Eletronic Library online (SCIELO). To carry out this research, we used the Descritores em Ciência da Saúde (DECS): periodontal diseases, dental scaling and cardiovascular diseases. The review demonstrated the fact that increases in CRP concentration are predictive of cardiovascular diseases (CVDs), and can be directly associated with the progress of atheromatous plaques formed in the carotid arteries. It also showed that untreated periodontitis is associated with possible alterations in lipid metabolism. In addition, lipid alterations in this context would be caused by infection that induces atherogenic changes in lipoprotein profiles. It was also seen that periodontal treatment performed correctly, works as a way to improve the clinical parameters of periodontal disease, as well as to favor the metabolic control of glucose in diabetic individuals, since periodontal disease can mean prognostic value for these cases. It is concluded that with this review there is evidence that non-surgical periodontal treatment can influence the decrease in levels of CRP, HbA1c and hyperlipidemia in patients with CVDs |

Como el presente estudio es una revisión de la literatura, para su producción, se consultaron las siguientes bases de datos: Biblioteca Virtual en Salud (BVS), Publicaciones Médicas (PubMed), Medline y Biblioteca electrónica en línea científica electrónica (SCIELO). Para llevar a cabo dicha investigación, se utilizaron los Descriptores de Ciencias de la Salud (DECS): enfermedades periodontales, escamas dentales y enfermedades cardiovasculares. La revisión demostró el hecho de que los aumentos en la concentración de PCR son predictivos de enfermedades cardiovasculares (ECV) y pueden estar directamente asociados con el progreso de las placas ateromatosas formadas en las arterias carótidas. También mostró que la periodontitis no tratada se asocia con posibles cambios en el metabolismo de los lípidos. Además, los cambios en los lípidos en esta área serían causados por la infección que induce cambios aterogénicos en los perfiles de lipoproteínas. También vio que el tratamiento periodontal realizado correctamente, funciona como una forma de mejorar los parámetros clínicos de la enfermedad periodontal, así como para favorecer el control metabólico de la glucosa en individuos diabéticos, ya que la enfermedad periodontal puede significar un valor pronóstico para estos casos. Con la presente revisión se concluye que hay evidencia de que el tratamiento periodontal no quirúrgico puede influir en la disminución de los niveles de PCR, HbA1c e hiperlipidemia en pacientes con ECV.O presente estudo analisou a literatura científica em busca de informações que possam contribuir para o conhecimento sobre a análise de índices que se julgam fatores de pré-disposição de problemas cardíacos após o tratamento periodontal não cirúrgico, mais especificamente perfil lipídico, hemoglobina glicada (HbA1c) e a proteína C reativa (PCR). Como o presente estudo se trata de uma revisão de literatura, para a sua produção, foram consultadas as bases de dados: Biblioteca Virtual em Saúde (BVS), Publicações Médicas (PubMed), Medline e biblioteca virtual Scientific Eletronic Library online (SCIELO). Para realizar tal pesquisa foram utilizados os Descritores em Ciência da Saúde (DECS): doenças periodontais, raspagem dentária e doenças cardiovasculares. A revisão demonstrou o fato das elevações na concentração de PCR serem preditivas de doenças cardiovasculares (DCVs), podendo ser diretamente associada ao progresso de placas ateromatosas formadas nas carótidas. Também evidenciou que a periodontite não tratada está sim, associada a possíveis alterações no metabolismo lipídico. Além disso, as alterações lipídicas nesse âmbito seriam causadas, pela infecção que induz modificações aterogênicas nos perfis de lipoproteínas. Viu também que o tratamento periodontal realizado corretamente, funciona como uma forma de aprimorar os parâmetros clínicos da doença periodontal, como também, de favorecer o controle metabólico da glicose em indivíduos diabéticos, visto que a doença periodontal pode significar valor prognóstico para esses casos.
