Ludwig´s Angina in the diabetic patient: evidence in literature

Resumo: Introduction: Ludwig’s Angina (LA) is a head and neck infection potentially fatal, which affects simultaneously submandibular, sublingual, and submental spaces. Diabetes mellitus (DM), due to its immunological and microvascular repercussions, can predispose to LA or modify the patient's response to this infection. Objective: to evaluate the impact of diabetes mellitus on patients with LA. Methods: This study is a literature review in which a search strategy was proposed that used MEDLINE as a database, accessed via PUBMED, limited to the English language and publications between 1954 to 2020. In the research, the following descriptors: “Ludwig's angina” and “diabetes”. Case reports and observational studies approach about Ludwig's angina in diabetic patients were included. Results: 26 articles were found and, after read the abstract, 12 articles were excluded, because there was no relation between LA and DM. Concerning to time of hospital stays, the patients diagnosed with hyperglycemia have a higher hospital stay, due to complications and clinical follow-up. The most common group of microorganisms isolated in diabetic patients was Klebsiella pneumoniae. The antibiotic therapy most commonly used empirically was an association with amoxicillin, clavulanate potassium and metronidazole. Conclusion: It’s possible to observe that Ludwig’s Angina had your clinical evolution modified for diabetes mellitus. The complications involving the association between DM and LA occurs as many diabetic patients without glycemic control, as diabetic patients controlled, to a lesser extent.

La diabetes mellitus (DM), por sus repercusiones inmunológicas y microvasculares, puede predisponer la AL o alterar la respuesta del paciente la essa infeccion. Objetivo: evaluar el impacto de la DM en pacientes con AL. Metodología: Esta es una revisión de la literatura en que se propuso una estrategia de búsqueda que utilizó MEDLINE como base de datos, a la que se accedió mediante del PUBMED, limitada al idioma inglés y publicaciones de 1954 a 2020. La investigación utilizó los siguientes descriptores: "angina de Ludwig" y "diabetes". Se incluyeron informes de casos y ensayos observacionales que abordaron la angina de Ludwig en pacientes diabeticos. Resultados: se encontraron 26 artículos y, después de leer sus resúmenes, se eliminaron 12 artículos que no establecieron ninguna relación entre AL y DM. En cuanto a la duración de la estancia hospitalaria, los pacientes diagnosticados con hiperglucemia pasan más días en el hospital, debido a complicaciones y seguimiento clínico. La bacteria Klebsiella pneumoniae se encontró en mayor número en el cultivo de pacientes diabéticos. El tratamiento farmacológico más utilizado empíricamente fue la asociación entre amoxicilina, clavulanato de potasio y metronidazol. Conclusión: es posible observar que la AL tiene su curso clínico modificado por la diabetes. Las complicaciones que involucran la asociación de DM y LA ocurren tanto en pacientes diabéticos sin control glucémico como en aquellos diabéticos controlados, aunque en menor medida.Introdução: Angina de Ludwig (AL) é uma infecção da cabeça e pescoço, potencialmente fatal, que acomete os espaços fáscias submandibulares, sublinguais e submentoniano. Objetivo: avaliar o impacto da diabetes melittus nos pacientes com AL. Metodologia Trata-se de uma revisão de literatura na qual foi proposta uma estratégia de busca que utilizou, como base de dados, o MEDLINE, acessado via PUBMED, limitada ao idioma inglês e a publicações do período de 1954 a 2020. Na pesquisa foram utilizados os seguintes descritores: “Ludwig’s angina” e “diabetes”. Incluíram-se relatos de caso e estudos observacionais que abordavam a angina de Ludwig em pacientes diabéticos. Resultados: foram encontrados 26 artigos e, após a leitura dos seus resumos, foram eliminados 12 artigos que não estabeleceram nenhuma relação entre AL e DM. Em relação ao tempo de internação hospitalar, os pacientes diagnosticados com hiperglicemia passam mais dias no hospital, devido a complicações e acompanhamento clínico. As bactérias do gênero Klebsiella pneumoniae foram encontradas em maior quantidade na cultura de pacientes diabéticos. A terapêutica medicamentosa mais utilizada empiricamente foi a associação entre amoxicilina, clavulanato de potássio e metronidazol. Conclusão: é possível observar que a Angina de Ludwig tem seu curso clínico modificado pela diabetes mellitus.
