Speech therapy practice to patients in palliative care: an integrative review

Resumo: The speech therapist can actively contribute within the multidisciplinary team of Palliative Care through assessment, prevention and rehabilitation, thus favoring the safety and efficiency of swallowing, communication, minimizing aspects that interfere in the quality of life of the subject under palliation. The objective of this study was to analyze the scientific production on speech therapy activities to patients in palliative care and discuss the strategies of speech therapy activities to patients in palliative care. This is an exploratory and descriptive research, consolidated an integrative literature review that aimed to analyze the scientific production on speech therapy to patients in palliative care. Once the descriptor associations were made in the databases, 25 articles were found and selected 06. After analyzing these articles, two categories were created: speech-language disorders in patients in Palliative Care; Speech therapy intervention strategies for patients in Palliative Care. Speech therapy can intervene, seeking the comfort and quality of the patient's pathway in palliative care and, also, providing a greater possibility of interaction between the patient, family, caregivers, information and health professionals through communication. As strategies that facilitate communication, the speech therapist can focus on direct therapy with the patient, the training of communication facilitators with the family and caregivers and the use of alternative means as a form of communication, such as: gestures and non-verbal communication, communication boards, computer systems and prostheses. In conclusion, speech therapy is important as a member of the multidisciplinary team in the care of patients in palliative care, since they can identify disorders that these patients may develop during treatment and which may lead to the insertion of this individual in a critical situation of terminality.

El objetivo de este estudio fue analizar la producción científica sobre las actividades de terapia del habla para pacientes en cuidados paliativos y discutir las estrategias de las actividades de terapia del habla para pacientes en cuidados paliativos. Esta es una investigación exploratoria y descriptiva, consolidó una revisión de literatura integradora que tuvo como objetivo analizar la producción científica sobre terapia del habla para pacientes en cuidados paliativos. Una vez que se hicieron las asociaciones de descriptores en las bases de datos, se encontraron 25 artículos y se seleccionaron 06. Después de analizar estos artículos, se crearon dos categorías: trastornos del habla y lenguaje en pacientes en Cuidados Paliativos; Estrategias de intervención de logopedia para pacientes en cuidados paliativos. La terapia del habla puede intervenir, buscando la comodidad y la calidad de la vía del paciente en cuidados paliativos y, además, brindando una mayor posibilidad de interacción entre el paciente, la familia, los cuidadores, los profesionales de la información y la salud a través de la comunicación. Como estrategias que facilitan la comunicación, el terapeuta del habla puede centrarse en la terapia directa con el paciente, la capacitación de facilitadores de comunicación con la familia y los cuidadores y el uso de medios alternativos como una forma de comunicación, tales como: gestos y comunicación no verbal, tableros de comunicación, sistemas informáticos y prótesis. En conclusión, la terapia del habla es importante como miembro del equipo multidisciplinario en la atención de pacientes en cuidados paliativos, ya que pueden identificar los trastornos que estos pacientes pueden desarrollar durante el tratamiento y que pueden conducir a la inserción de este individuo en una situación crítica de terminalidad.O fonoaudiólogo pode contribuir ativamente dentro da equipe multiprofissional de Cuidados Paliativos por meio da avaliação, prevenção e reabilitação, favorecendo assim a segurança e eficiência da deglutição, comunicação, minimizando aspectos que interferem na qualidade de vida do sujeito em paliação. Trata-se de uma pesquisa exploratória e descritiva, consolidada em revisão integrativa de literatura que objetivou em analisar a produção científica sobre a atuação fonoaudiológica a pacientes em cuidados paliativos. Feitas as associações dos descritores nas bases de dados, foram encontrados 25 artigos e selecionados 06. Após análise dos referidos artigos foram criadas duas categorias: distúrbios fonoaudiológicos em pacientes em Cuidados Paliativos; estratégias de intervenção Fonoaudiológica frente a pacientes em Cuidados Paliativos. A fonoaudiologia pode intervir, buscando o conforto e qualidade de vida do paciente em cuidados paliativos e, também, propiciar uma maior possibilidade de interação entre o paciente, familiares, cuidadores informais e profissionais de saúde através da comunicação. Como estratégias que facilitam a comunicação, o fonoaudiólogo pode atentar a terapia direta com o paciente, o treino de facilitadores de comunicação com a família e cuidadores e a utilização de meios alternativos como forma de comunicação, como por exemplo: gestos e comunicação não verbais, pranchas de comunicação, sistemas computadorizados e próteses.
