Application of botulinum toxin as a therapeutic measure for bruxism and gummy smile: a literature review

Resumo: Produced by the bacterium Clostridium botulinum, the application of botulinum toxin in dental procedures is increasing and targeted by patients, this because it presents effective protocols and safe techniques. Therefore, the present study aimed to carry out a literature review about the application of botulinum toxin as a therapeutic measure for bruxism and gummy smile. To this end, searches were carried out on PubMed, Scientific Electronic Library Online and Virtual Health Library platforms using the descriptors: “Botulinum Toxins, Type A”, “Dental Care” and “Esthetics”. Bruxism is an oral disorder that may involve multiprofessional treatment and is characterized by clenching or grinding the teeth, pushing or not the jaw, which occurs during the day or night, affecting the stomatognathic system. The literature has shown that botulinum toxin is an excellent alternative in the treatment not only for aesthetics, but also in cases of bruxism and gummy smile. It is a gingival smile when, in the act of smiling, there is exposure of the gingiva greater than three millimeters, with a higher prevalence in women. The effect of the drug is individualized, varying according to the patient, however it is guaranteed to be safe and with a high rate of clinical success. Therefore, the professional must have knowledge about facial anatomy and substance application techniques.

Por ello, el presente estudio tuvo como objetivo realizar una revisión bibliográfica acerca de la aplicación de la toxina botulínica como medida terapéutica para el bruxismo y la sonrisa gingival. Para ello, se realizaron búsquedas en las plataformas PubMed, Scientific Electronic Library Online y Virtual Health Library utilizando los descriptores: “Botulinum Toxins, Type A”, “Dental Care” y “Esthetics”. El bruxismo es un trastorno bucal que puede implicar un tratamiento multidisciplinario y se caracteriza por apretar o rechinar los dientes, empujar o no la mandíbula, que se presenta durante el día o la noche, afectando el sistema estomatognático. La literatura ha demostrado que la toxina botulínica es una excelente alternativa en el tratamiento no solo estético, sino también en casos de bruxismo y sonrisa gingival. Es una sonrisa gingival cuando, en el acto de sonreír, existe una exposición de la encía superior a tres milímetros, con mayor prevalencia en mujeres. El efecto del fármaco es individualizado, variando según el paciente, sin embargo se garantiza que es seguro y con una alta tasa de éxito clínico. Por tanto, el profesional debe tener conocimientos sobre anatomía facial y técnicas de aplicación de sustancias.Produzida pela bactéria Clostridium botulinum, a aplicação da toxina botulínica em procedimentos odontológicos é crescente e visada pelos pacientes, isso por apresentar protocolos eficazes e técnicas seguras. Sendo assim, o presente estudo visou realizar uma revisão bibliográfica acerca da aplicação da toxina botulínica como medida terapêutica para bruxismo e sorriso gengival. Para tal, realizaram-se buscas nas plataformas PubMed, Scientific Eletronic Library Onlinee Biblioteca Virtual em Saúde utilizando os descritores: “Botulinum Toxins, Type A”, “Dental Care”e“Esthetics”. O bruxismo é uma alteração oral que pode envolver tratamento multiprofissional e tem como característica o apertamento ou ranger os dentes, empurrando ou não a mandíbula, que ocorre durante o período diurno ou noturno, afetando o sistema estomatognático. A literatura demonstrou que a toxina botulínica é uma excelente alternativa no tratamento não só para estética, mas também em casos de bruxismo e sorriso gengival. Trata-se de sorriso gengival quando no ato de sorrir, há exposição da gengiva maior que três milímetros, sendo a prevalência maior em mulheres. O efeito do fármaco é individualizado, variando conforme o paciente, contudo é garantido como seguro e com alta taxa de sucesso clínico.
