Botulinum toxin and Dentistry: integrative review

Resumo: Introduction: Botulinum toxin has an important therapeutic application. Produced by the bacterium Clostridium botulinum, it gained great prominence in the dental sector for having safe, fast and effective protocols. Objectives: To compile and synthesize updated information about the use of botulinum toxin in dentistry. Methodology: This is na integrative literature review. For data collection, searches were performed on PubMed, Scientific Electronic Library Online and Virtual Health Library platforms by consulting the descriptors: “Botulinum Toxins, Type A”, “Dental Care” and “Esthetics”. Complete studies in English, Portuguese and Spanish published in the period from 2012 to 2022 were selected. Results: 55 articles stood out, whose analysis allowed us to identify that the drug is effective as an alternative treatment in aesthetic and functional/pathological cases. As examples, we can mention the correction of facial asymmetries, combat and attenuation of various types of wrinkles and treatment of tension headache, temporomandibular disorder, bruxism, masseter hypertrophy and trigeminal neuralgia. Conclusion: The effects of botulinum toxin application are not definitives. Therefore, its administration causes temporary chemical denervation. The success of the technique mainly depends on the correct identification of the muscle(s) involved in the problem, on the absence of general muscle hyperactivity in the area of administration and on possible iatrogenic causes. Thus, the toxin proves to be effective in cases where the current protocols and the limitations of the chosen therapy are respected.

Producida por la bacteria Clostridium botulinum, no obtuvo gran protagonismo el sector odontológico por protocolos seguros, rápidos y eficientes. Objetivos: Analizar y sintetizar información actualizada sobre el uso de la toxina botulínica en Odontología. Metodología: Esta es una revisión integradora. Para la recolección de datos, se realizaron búsquedas en las plataformas PubMed, Scientific Electronic Library Online y Virtual Health Library consultando los descriptores: “Botulinum Toxins, Type A”, “Dental Care” y “Esthetics”. Se seleccionaron estúdios completos en inglés, portugués y español publicados entre 2012 y 2022. Resultados: De los 698 artículos examinados, se destacaron 55, cuyo análisis permitió identificar que el fármaco es eficaz como alternativa de tratamiento en casos estéticos y funcionales/patológicos. Algunos ejemplos son: corrección de asimetrías faciales, combate y atenuación de diversos tipos de arrugas y tratamiento de cefalea tensional, transtorno temporomandibular, bruxismo hipertrofia del masetero y neuralgia del trigémino. Conclusión: Los efectos de la aplicación de toxina botulínica son transitorios. Es decir, su administración provoca una denervación química temporal. El éxito de la técnica depende estrechamente, entre otras cuestiones, de la correcta identificación del músculo o músculos implicados en el problema, de la ausencia de hiperactividad muscular general en la zona afectada y de posibles causas iatrogénicas. Así, la toxina demuestra ser eficaz en los casos en que se respetan los protocolos vigentes y las limitaciones de la terapia de elección.Introdução: A toxina botulínica tem sua aplicação magistral no âmbito terapêutico. Produzida pela bactéria Clostridium botulinum, obteve grande destaque no setor odontológico por possuir protocolos seguros, rápidos, eficazes e replicáveis. Objetivos: Analisar e sintetizar informações atualizadas acerca do uso da toxina botulínica no âmbito da Odontologia. Metodologia: Trata-se de uma revisão integrativa. Para coleta de dados foram realizadas buscas nas plataformas PubMed, Scientific Eletronic Library Online e Biblioteca Virtual em Saúde mediante a consulta dos descritores: “Botulinum Toxins, Type A”, “Dental Care”e “Esthetics”. Foram selecionados estudos completos em inglês, português e espanhol publicados no período de 2012 a 2022. Resultados: Dos 698 artigos examinados, destacaram-se 55 cuja análise permitiu identificar que o fármaco é eficaz como alternativa de tratamento em casos estéticos e funcionais/patológicos. São citados como exemplos: correção de assimetrias faciais, combate e atenuação de vários tipos de rugas e tratamento da cefaleia tensional, disfunção temporomandibular, bruxismo, hipertrofia de masseter e neuralgia de trigêmeo. Conclusão: Os efeitos da aplicação da toxina botulínica são transitórios. Ou seja, sua administração causa denervação química temporária. O sucesso da técnica depende intimamente, além de outras questões, da correta identificação do(s) músculo(s) envolvido(s) no problema, da inexistência de hiperatividade geral muscular na área em questão e de possíveis causas iatrogênicas.
