The effects of prone positioning on the respiratory mechanics of patients with Covid-19 at the intensive care center: a systematic review

Resumo: Corona Virus Disease-19 (Covid-19) is a disease caused by SARS-CoV-2, which has reached the status of a pandemic and has dramatically increased the need for admissions to Intensive Care Units (ICU's). In this context, the use of techniques for the support and care of the sick highlighted the prone position, a conduct on which several studies elucidate the physiological benefits for the respiratory system. To investigate the effects of applying prone positioning in the intensive care setting in patients with Covid-19, exploring the benefits and pointing out the possible counterpoints of this therapy. Bibliographic survey in the VHL (MEDLINE and LILACS) and PUBMED databases. The study included 13 articles in Portuguese and English, published in the last 5 years. Development: Most studies analyzed confirm the feasibility and safety of applying prone positioning in patients with Covid-19, where there is considerable improvement in peripheral oxygenation and ventilation-perfusion ratio, although some studies have not found significant results. It is concluded that the technique is safe and effective and it is noted that the pronation period and patient adherence to therapy, when awake, can be a determining factor for treatment success.

En este contexto, el uso de técnicas para el apoyo y cuidado de los enfermos destacó la posición prona, una conducta sobre la que varios estudios dilucidan los beneficios fisiológicos para el sistema respiratorio. Investigar los efectos de la aplicación del decúbito prono en cuidados intensivos en pacientes con Covid-19, explorando los beneficios y señalando los posibles contrapuntos de esta terapia. Metodología: Encuesta bibliográfica en las bases de datos BVS (MEDLINE y LILACS) y PUBMED. El estudio incluyó 13 artículos en portugués e inglés, publicados en los últimos 5 años. La mayoría de los estudios analizados confirman la viabilidad y seguridad de la aplicación del decúbito prono en pacientes con Covid-19, donde hay una mejora considerable en la oxigenación periférica y la relación ventilación-perfusión, aunque algunos estudios no han encontrado resultados significativos. Se concluye que la técnica es segura y efectiva y se observa que el período de pronación y la adherencia del paciente a la terapia, cuando está despierto, puede ser un factor determinante para el éxito del tratamiento.A Corona Vírus Disease-19 (Covid-19) é uma doença provocada pelo SARS-CoV-2, que alcançou o status de pandemia e aumentou vertiginosamente a necessidade de internações nos Centro de Terapia Intensivas (CTI's). Nesse contexto, o emprego de técnicas para o suporte e cuidados dos enfermos colocou em evidência o posicionamento em prono, conduta sobre a qual vários estudos elucidam quanto aos benefícios fisiológicos para o sistema respiratório. Investigar sobre os efeitos da aplicação do posicionamento em prono no ambiente de terapia intensiva, em pacientes com Covid-19, explorando os benefícios e apontando os possíveis contrapontos desta terapêutica. Levantamento bibliográfico nas bases de dados BVS (MEDLINE e LILACS) e PUBMED. Foram incluídos no estudo 13 artigos nos idiomas português e inglês, publicados nos últimos 5 anos. A maioria dos estudos analisados corrobora a viabilidade e segurança da aplicação do posicionamento em prono em pacientes com Covid-19, onde observa-se melhora considerável da oxigenação periférica e relação ventilação-perfusão, embora algumas pesquisas não tenham encontrado resultados significativos.
