Opportunistic oral infections in patients submitted to radiotherapy for head and neck cancer: a retrospective study

Resumo: To evaluate retrospectively the prevalence of opportunistic oral infections in head and neck cancer patients submitted to radiotherapy, in combination or not with chemotherapy and surgery. The survey was performed by revision of electronic medical records of patients submitted to 3D-conformational RT for head and neck cancer from the Record Book of Medical Physics of Radiotherapy Division at Federal University of São Paulo (São Paulo Hospital), considering personal and clinical data. Among the 79 patients evaluated, 38 developed an opportunistic oral infection. Besides, most of them were white men, younger than 60 years, ex-smokers, ex-alcoholic, and underwent dental treatment pre-RT. The most frequent histological type was the squamous cell carcinoma and the most affected anatomical site was the larynx. There were also higher rates of stage IV tumors and oncologic treatments performed with a combination of RT, chemotherapy, and surgery. In relation to the opportunistic oral infections, candidiasis was the most representative. Patients with advanced-stage head and neck cancer seem to present oral candidiasis more frequently, probably due to the multimodal treatment.

La recopilación de datos se realizó mediante la revisión de los registros médicos electrónicos de pacientes sometidos a RT conformacional 3D de la cabeza y el cuello que estaban en el Libro de registro de física médica del Sector de Radioterapia de la Universidad Federal de São Paulo (Hospital São Paulo), considerando la información individual y clinico. De los 79 pacientes evaluados, 38 desarrollaron alguna infección oral oportunista. Entre estos, hubo una prevalencia de leucodermia masculina, de menos de 60 años y antecedentes de tabaquismo, alcoholismo y tratamiento dental pre-RT. Predominó el carcinoma de células escamosas como tipo histológico, la laringe como sitio anatómico, el estadio IV y el tratamiento del cáncer realizado con una combinación de radioterapia, quimioterapia y cirugía. Con respecto a las infecciones orales oportunistas, la candidiasis fue la más representativa. Los pacientes con neoplasias malignas de la cabeza y el cuello en estadio avanzado parecen presentar candidiasis oral con mayor frecuencia, probablemente debido al tratamiento multimodal.Avaliar retrospectivamente a prevalência de infecções orais oportunistas em pacientes portadores de neoplasias malignas de cabeça e pescoço submetidos a radioterapia (RT), associada ou não a quimioterapia e/ou cirurgia. Levantamento de dados foi realizado por meio de revisão dos prontuários eletrônicos dos pacientes submetidos à RT conformacional 3D de cabeça e pescoço que constavam no Livro de Registro da Física Médica do Setor de Radioterapia da Universidade Federal de São Paulo (Hospital São Paulo), considerando informações individuais e clínicas. Dos 79 pacientes avaliados, 38 desenvolveram alguma infecção oral oportunista. Entre estes, observou-se prevalência de leucodermas do sexo masculino, com idade inferior a 60 anos e história de tabagismo, etilismo e tratamento odontológico pré-RT. Houve predomínio do carcinoma espinocelular como tipo histológico, da laringe como sítio anatômico, do estádio IV e do tratamento oncológico realizado com combinação de radioterapia, quimioterapia e cirurgia. Com relação às infecções orais oportunistas, a candidose foi a mais representativa.
