Wound treatment using low-level laser as adjuvant therapy: an integrative literature review

Resumo: This paper is an integrative literature review, under descriptive qualitative design. Its goal is to evaluate the parameters of low intensity laser’s dosimetry in the treatment of lesions caused by pressure and diabetes-related ulcers more largely utilized on scientific evidences. Data was collected from Latin American and Caribbean-based literature available on Ciencias da Saude and Base de Dados de Enfermagem, through Biblioteca Virtual em Saúde; Medical Literature Analysis and Retrieval System Online, through PubMed; Web of Science; Cumulative Index of Nursing and Allied Health and EMBASE, filtered by date from 2016 to 2022. The following controlling and non-controlling key-words were used: low level lighttherapy; LLLT; laser biostimulation, laser therapy, photobiomdulation, wounds and injuries; wound healing; diabetic foot; pressure ulcer. Results: 14 studies were analyzed according to two thematic categories, so an evaluation of the parameters of the Low Intensity Laser used on lesions could be made, according to the following etiologies: diabetic wound and lesion by pressure. Conclusion: All the studies found a positive outlook regarding the use of low intensity laser on the treatment of diabetic-related lesions or the ones caused by pressure. The majority recommended a specific length of red range (558nm-660nm). There were discrepancies regarding the energy applied. Nevertheless, the majority of studies present a window from 3 J/cm2 to 6 J/cm2. The category destined to study diabetes-caused ulcers had the potency of 30mW, on the wide majority of cases. The ones related to lesions caused by pressure showed good results with potencies from 3,6mW to 100mW.

El objetivo de este artículo es evaluar los parámetros relacionados con la dosimetria láser de baja intensidade en el tratamiento de las úlceras por presión y las úlceras diabéticas más utilizados en la evidencia científica. Los datos fueron recolectados de las Bases de Datos de Literatura Latinoamericana y del Caribe en Ciencias de la Salud y Enfermería, por la Biblioteca Virtual en Salud; Sistema de Recuperación y Análisis de Literatura Médica en línea, por la PubMed; Web of Science; Cumulative Index of Nursing and Allied Health y EMBASE, utilizando el filtro temporal de 2016 a 2022. Se utilizaron los siguientes descriptores controlados y no controlados: fototerapia de bajo nível; LLLT; bioestimulacíon con láser; terapia con láser; fotobiomodulacíon; heridas y lesiones; cicatrización de la helidas; pie diabético; úlcera de presión. Resultados: se analizaron 14 estudios basados en dos categorías temáticas para evaluar los parámetros de Laser de Baja Intensidad utilizados em lesiones con las siguientes etiologias: herida diabética y lesión por presión. Conclusión: Todos los estúdios analizados encontraron un resultado positivo con el uso de lser de bajo nível en el tratamiento de lesión por presión o úlceras diabéticas. La mayoría de los estudios recomiendó longitudes de onda em el rango rajo (658 nm – 660 nm). Hubo una gran discrepancia en los hallazgos relacionados con la energía aplicada, pero la mayoría de los estudios presentan uma ventana de 3 J/cm2 a 6 J/cm2. La categoría que analizó los estudios con úlcera diabética tuvo una potencia de 30 mW en la mayoría de los artículos. La categoria de lesiones por presión obtuvo buenos reultados con potencias desde 3,6 mW hasta 100 mW.Trata-se de uma revisão integrativa de literatura com abordagem qualitativa. O objetivo deste artigo é avaliar os parâmetros relacionados à dosimetria do laser de baixa intensidade no tratamento de lesões por pressão e úlceras diabéticas mais utilizados nas evidências científicas. Dados foram coletados nas bases Literatura Latino-Americana e do Caribe em Ciências da Saúde e Base de Dados de Enfermagem, pela Biblioteca Virtual em Saúde; Medical Literature Analysis and Retrieval System Online, pela PubMed; Web of Science; Cumulative Index of Nursing and Allied Health e EMBASE, utilizando-se o filtro temporal de 2016 a 2022. Foram usados os seguintes descritores controlados e não controlados: low level light therapy; LLLT; laser biostimulation; laser therapy; photobiomodulation; wounds and injuries; wound healing; diabetic foot; pressure ulcer. Resultados: Foram analisados 14 estudos a partir de duas categorias temáticas para avaliação dos parâmetros do laser de baixa intensidade utilizados nas lesões com as seguintes etiologias: ferida diabética e lesão por pressão. Conclusão: Todos os estudos encontraram desfecho positivo com uso de laser de baixa intensidade no tratamento de lesões por pressão ou úlceras diabéticas. A maioria recomendou comprimento de onda na faixa do vermelho (658 nm – 660 nm). Houve grande discrepância quanto à energia aplicada, porém a maioria dos trabalhos apresentou uma janela de 3 J/cm2 a 6 J/cm2. A categoria que analisou os estudos com úlcera diabética teve potência de 30 mW na maioria dos artigos.
