Relation of the vasoconstrictor present in the local anesthetics in patients with heart disease

Resumo: The need for more invasive dental procedures in patients with heart disease has been growing and the administration of local anesthetics associated with vasoconstrictors is the most used method in order to allow the necessary adaptation for a range of conducts. Among these, tooth extraction is one of the most frequent procedures in the dentist's daily life and despite the low rate of complications, treatment planning must consider factors such as: age, gender and medical and dental history. Constant monitoring of vital signs and knowledge of the anesthetic volume and appropriate technique for each case is essential to reduce complications such as anesthetic intoxication, arrhythmias, heart failure, significant changes in systolic and diastolic pressures and hypersensitivity reactions. The manuscript is a review of narrative literature with a qualitative approach, this aimed to understand the immediate effects of vasoconstrictors present in local anesthetic in patients with heart disease. The search was performed on the VHL, PubMed, Scielo and Google Scholar platforms, written in Portuguese and English, between 2007 and 2022. Studies report that at certain dosages there are no significant clinical differences and the use of vasoconstrictors is not contraindicated in patients with cardiovascular diseases. However, there is not enough scientific evidence to say that the possible side effects in patients with heart disease are exclusively due to vasoconstrictors.

Entre estos, la extracción dental es uno de los procedimientos más frecuentes en el día a día del odontólogo y, a pesar de la baja tasa de complicaciones, la planificación del tratamiento debe considerar factores como: edad, género e historial médico y odontológico. El monitoreo constante de los signos vitales y el conocimiento del volumen anestésico y la técnica adecuada para cada caso es fundamental para reducir complicaciones como intoxicaciones anestésicas, arritmias, insuficiencia cardíaca, cambios significativos en las presiones sistólica y diastólica y reacciones de hipersensibilidad. El manuscrito es una revisión de la literatura narrativa con un enfoque cualitativo, esto tuvo como objetivo comprender los efectos inmediatos de los vasoconstrictores presentes en los anestésicos locales en pacientes con enfermedades del corazón. La búsqueda se realizó en las plataformas de la BVS, PubMed, Scielo y Google Scholar, escrita en portugués e inglés, entre 2007 y 2022. Estudios informan que a determinadas dosis no existen diferencias clínicas significativas y el uso de vasoconstrictores no está contraindicado en pacientes con enfermedades cardiovasculares. Sin embargo, no hay suficiente evidencia científica para decir que los posibles efectos secundarios en pacientes con enfermedades del corazón se deban exclusivamente a los vasoconstrictores.              A necessidade de procedimentos odontológicos mais invasivos em pacientes cardiopatas tem sido crescente e a administração de anestésicos locais associados a vasoconstritores é o método mais utilizado no sentido de permitir a adequação necessária para uma gama de condutas. Entre estas, a exodontia é um dos procedimentos mais frequentes no cotidiano do cirurgião-dentista e apesar da baixa taxa de complicações, o planejamento do tratamento deve considerar fatores como: idade, gênero e histórico médico e odontológico. A monitorização constante dos sinais vitais e conhecimento do volume anestésico e técnica adequada para cada caso é imprescindível para diminuir intercorrências como intoxicação anestésica, arritmias, insuficiência cardíaca, alterações significativas nas pressões sistólica e diastólica e reações de hipersensibilidade. O manuscrito trata-se de uma revisão de literatura narrativa com abordagem qualitativa, este objetivou compreender sobre os efeitos imediatos dos vasoconstrictores presentes no anestésico local em pacientes cardiopatas. A busca foi realizada nas plataformas BVS, PubMed, Scielo e Google Acadêmico redigidos nos idiomas português e inglês, entre 2007 e 2022. Os estudos relatam que em determinadas dosagens não há diferenças clínicas significativas e o uso de vasoconstritores não é contra-indicado em pacientes com doenças cardiovasculares.
