Factors associated with falls in elderly inpatients: an integrative review

Resumo: | Objective: To identify the main factors associated with falls in elderly inpatients in the scientific literature. Method: We conducted an integrative literature review with searches in the following databases: MedLine via VHL, PubMed, SciELO, Web of Science, Scopus, and LILACS. We identified 2,969 publications, of which 11 were selected for the final sample of this study. We built a theoretical model for the critical analysis and data presentation. The factors that presented association or risk for falls in elderly inpatients were considered as evidence. Results: The factors associated with falls in elderly inpatients were categorized into intrinsic and extrinsic. The main intrinsic factors identified were: 1) functional decline, delirium, history of falls, and advanced age; and the extrinsic factors: admission to a surgical ward and the use of central nervous system depressants, more precisely sedatives and antipsychotics. We observed that these factors are associated in the studied population. Conclusion: The studies showed that intrinsic factors are the main risk factors for falls in hospitalized elderly. However, we observed their association with extrinsic factors and the complexity of falls. Thus, these findings highlight the importance of multidimensional assessment of hospitalized elderly, the adoption of fall prevention strategies and timely intervention on modifiable factors, providing more safety to the elderly patient in the hospital environment. |

Método: Estudio de revisión integradora de la literatura, con búsquedas en las bases de datos: MedLine vía BVS, PubMed, SciELO, Web of Science, Scopus y LILACS. De las 2.969 publicaciones identificadas, 11 se incluyeron en la muestra final de este estudio. Se construyó un modelo teórico para apoyar el análisis crítico y la presentación de los datos. Este estudio consideró como evidencia, los factores que presentaron asociación o riesgo para la ocurrencia de caídas en ancianos hospitalizados. Resultados: Los factores asociados a la aparición de caídas en pacientes ancianos hospitalizados se clasificaron en intrínsecos y extrínsecos. Los principales factores intrínsecos identificados fueron: 1) el deterioro funcional, el delirio, los antecedentes de caídas y la edad avanzada; y los factores extrínsecos: el ingreso en una sala de cirugía y el uso de depresores del sistema nervioso central, más concretamente sedantes y antipsicóticos. Se observó que estos factores están asociados en la población estudiada. Conclusión: Los estudios evidenciaron que los factores intrínsecos son los principales factores de riesgo de caídas en los ancianos hospitalizados. Sin embargo, se observa su asociación con factores extrínsecos y la complejidad de este evento. Por lo tanto, estos resultados destacan la importancia de la evaluación multidimensional de los ancianos hospitalizados, la adopción de estrategias de prevención de caídas y la intervención oportuna sobre los factores modificables, proporcionando una mayor seguridad a los pacientes ancianos en el entorno hospitalario.Objetivo: Identificar na literatura científica os principais fatores associados a quedas em pessoas idosas hospitalizadas. Este estudo considerou como evidência, os fatores que apresentaram associação ou risco para ocorrência de quedas em idosos hospitalizados. Resultados: Os fatores associados a ocorrência de quedas em pacientes idosos hospitalizados foram classificados em intrínsecos e extrínsecos. Os principais fatores intrínsecos identificados foram: 1) declínio funcional, delirium, histórico de quedas e idade avançada; e os extrínsecos: internação em enfermaria cirúrgica e o uso de depressores do sistema nervoso central, mais precisamente sedativos e antipsicóticos. Observou-se que estes fatores se apresentam de forma associada na população estudada. Conclusão: Os estudos evidenciaram os fatores intrínsecos como os principais fatores de risco para ocorrência de quedas em idosos hospitalizados. Porém, observa-se sua associação com os fatores extrínsecos e a complexidade deste evento.
