People with disabilities and barriers to health services: a metasynthesis approach

Resumo: The objective of this paper as a literature review is to identify and analyze the barriers that influence access to health services and care by people with disabilities, using the qualitative metasynthesis approach. To do so, it aimed a sweep in the BVS and MEDLINE/PubMed databases, finding 31 articles. After the textual scanning, it described and categorized the challenges identified as physical barriers, transportation barriers, economic and social barriers, technological barriers, communication and information barriers, organizational and political barriers, care barriers, attitudinal barriers, and cultural and symbolic barriers. The results pointed out challenges in mobility and the lack of services and professionals to welcome and support people with disabilities. As for the barriers, it highlights the absence of technologies for assessment and accessible diagnosis and the shortage of human resources and assistive technology to meet the demands and provide qualified care. The behaviors, attitudes and cultural and symbolic values influenced the relationship between people with disabilities to health services and professionals and their choice to seek care. The results of this review can help managers and care professionals on the importance of adopting inclusive practices and encourage researches that aim for new perspectives and propositional solutions.

Se buscaron estudios en las bases de datos BVS y MEDLINE/PubMed, siendo seleccionados 31 artículos. Los desafíos identificados se han descripto y clasificados en barreras físicas, barreras de transportes, barreras socioeconómicas, barreras tecnológicas, barreras en la comunicación y la información, barreras institucionales y políticas, barreras asistenciales, barreras actitudinales, barreras culturales y simbólicas. Los resultados apuntan a los retos de la movilidad y la falta de preparación de los servicios y de los profesionales para recibir y atender las personas con discapacidad. En los obstáculos se fue identificado la ausencia de tecnologías para evaluación y diagnóstico asequibles, la falta de recursos humanos, tecnología de asistencia para satisfacer las demandas y prestar asistencia de forma cualificada. Los comportamientos, actitudes, valores culturales y simbólicos han influido principalmente en la relación de las personas con discapacidad, con los servicios de salud y sus profesionales y con la elección de buscar atención. Los resultados de esta revisión pueden servir de apoyo a los gestores y a los profesionales de la sanidad sobre la importancia de adoptar prácticas inclusivas y fomentar nuevas investigaciones destinadas a buscar nuevas perspectivas y soluciones positivas.O objetivo desta revisão de literatura foi identificar e analisar as barreiras que influenciam no acesso aos serviços e aos cuidados de saúde pelas pessoas com deficiência, por meio da abordagem da metassíntese qualitativa. Buscaram-se estudos nas bases de dados BVS e MEDLINE/PubMed, sendo selecionados 31 artigos. Os desafios identificados foram descritos e categorizados em barreiras físicas, barreiras nos transportes, barreiras econômico-sociais, barreiras tecnológicas, barreiras nas comunicações e na informação, barreiras organizacionais e políticas, barreiras assistenciais, barreiras atitudinais e barreiras culturais e simbólicas. Os resultados apontaram desafios na mobilidade e no despreparo dos serviços e dos profissionais para receber pessoas com deficiência e dar assistência a elas. Nas barreiras, identificaram-se a ausência de tecnologias para avaliação e diagnóstico acessíveis e a falta de recursos humanos e de tecnologia assistiva para suprir as demandas e prover os atendimentos de maneira qualificada. Os comportamentos, atitudes e valores culturais e simbólicos influenciaram, principalmente, na relação das pessoas com deficiência com os serviços de saúde e seus profissionais e com a escolha de buscar pelo cuidado.
