The practice of breastfeeding among women working: an integrative review

Resumo: Objective: To analyze the scientific evidence about the right to breastfeeding among women working. Methods: Integrative literature review guided by the question: What aspects are evidenced about the practice of breastfeeding among women working? Searches were conducted in the LILACS, PubMed® and MEDLINE® databases, and in the SciELO electronic library in May 2019, using the descriptors “Breast feeding”, “Women working”, “Workplace”, “Legislation, labor” combined using the Boolean operators “AND” and “OR”. Studies available in full in Portuguese, English and Spanish were included, excluding dissertations, theses, literature reviews and editorial notes. Results: The final sample resulted in 15 articles, most of the studies were carried out in different international settings and with level of evidence VI. The largest proportion of studies used a cross-sectional design. Factors such as maternal work, maternal education, low family income, maternal age, type of delivery, maternal decision (mother no longer wants to breastfeed), depression and parity were associated with early weaning. The lack of supportive work environments, lactation facilities and the period of paid maternity leave were all cited as barriers to breastfeeding in the workplace. Conclusion: Studies have shown the difficulty in maintaining breastfeeding after returning to work. Rights related to the promotion of breastfeeding that are not guaranteed in their entirety make it impossible to continue exclusive breastfeeding among women working. The extension of maternity leave from 120 to 180 days is identified as the main factor that reduces the weaning rate.

Métodos: Revisión integrativa de la literatura guiada por la pregunta: ¿Qué aspectos se evidencian sobre la práctica de la lactancia materna entre las mujeres trabajadoras? Las búsquedas se realizaron en las bases de datos LILACS, PubMed® y MEDLINE®, y en la biblioteca electrónica SciELO en mayo de 2019, utilizando los descriptores “Lactancia materna”, “Mujeres que trabajan”, “Lugar de trabajo”, “Legislación, trabajo” combinados utilizando el Operadores booleanos "Y" y "O". Se incluyeron estudios disponibles íntegramente en portugués, inglés y español, excluyendo disertaciones, tesis, revisiones de literatura y notas editoriales. Resultados: La muestra final resultó en 15 artículos, la mayoría de los estudios se realizaron en diferentes escenarios internacionales y con nivel de evidencia VI. La mayor proporción de estudios utilizó un diseño transversal. Factores como el trabajo materno, la educación de la madre, los bajos ingresos familiares, la edad materna, el tipo de parto, la decisión materna (la madre ya no quiere amamantar), la depresión y la paridad se asociaron con el destete temprano. La falta de entornos laborales favorables, instalaciones para la lactancia y el período de licencia por maternidad remunerada se mencionaron como obstáculos para la lactancia materna en el lugar de trabajo. Conclusión: los estudios han demostrado la dificultad de mantener la lactancia materna después de regresar al trabajo. Los derechos relacionados con la promoción de la lactancia materna que no están garantizados en su totalidad imposibilitan la continuación de la lactancia materna exclusiva entre las mujeres trabajadoras. La extensión de la baja por maternidad de 120 a 180 días se identifica como el principal factor que reduce la tasa de destete.Objetivo: Analisar as evidências científicas acerca do direito ao aleitamento materno entre mulheres em atividade laboral. Métodos: Revisão integrativa da literatura norteada pela questão: Quais os aspectos evidenciados acerca da prática ao aleitamento materno entre mulheres em atividade laboral? Buscas realizadas nas bases de dados LILACS, PubMed® e MEDLINE® e SciELO em maio de 2020, utilizando os descritores “Breast feeding”, “Women working”, “Workplace”, “Legislation, labor” combinados utilizando os operadores booleanos “AND” e “OR”. Incluíu-se estudos disponíveis na íntegra, em português, inglês e espanhol, sendo excluídas dissertações, teses, revisões de literatura e notas editoriais.  Resultados: A amostra final resultou em 15 artigos, a maioria dos estudos foram realizados em distintos cenários internacionais e com nível de evidência VI. A maior proporção de estudos utilizou delineamento transversal.  Fatores como trabalho materno, escolaridade materna, renda familiar baixa, idade materna, tipo de parto, decisão materna, depressão e paridade estiveram associados ao desmame precoce. A falta de ambientes de apoio no trabalho, as instalações de lactação e o período de licença-maternidade remunerada foram todos citados como barreiras à amamentação no local de trabalho. Conclusão: Os estudos demonstraram a dificuldade de manutenção do aleitamento materno após o retorno ao trabalho. Direitos relacionados à promoção do aleitamento não garantidos em sua totalidade impossibilitam a continuidade do aleitamento materno exclusivo entre mulheres em atividade laboral.
