Impact of the implementation of the birthing centers on the quality of obstetric care

Resumo: Objective: To describe the effects that the implementation of the Birthing Centers (BC) had on the quality of obstetric care according to the maternal and neonatal results. Methods: Integrative Literature Review, whose articles were searched in the Scientific Eletronic Library Online (SciELO), Medical Literature Analysis and Retrievel System Online (Medline) and Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS) databases. Results: Just over two decades since the beginning of the implementation of BC in Brazil, studies show positive impacts of this system in the care of parturients and newborns. Nulliparity without associated comorbidities, presence of companions and use of non-lithotomic delivery positions were present in the studies analyzed, positively influencing parturients outcomes: lower use of oxytocin, decreased episiotomy, Apgar index higher than 7 and a few reported cases of maternal and fetal removals. Final Considerations: It is concluded that the adoption of BC in the care network for pregnant women is conducive to maternal and neonatal care, not excluding hospital care for states that requires more complex care, as well as support in emergency situations.

Métodos: Revisión Integral de Literatura, cuyos artículos fueron buscados en las bases de datos Scientific Eletronic Library Online (SciELO), Medical Literature Analysis and Retrievel System Online (Medline) y Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS). Resultados: Con poco más de dos décadas desde el comienzo de la implementación de los ANC en Brasil, los estudios muestran impactos positivos de este sistema en el cuidado de los recién nacidos y las parturientas. La nuliparidad sin comorbilidades asociadas, la presencia de acompañantes y el uso de posiciones de parto no litotómicas estuvieron presentes en los estudios analizados, lo que influyó positivamente en los resultados de las parturientas: menor uso de oxitocina, disminución del rendimiento de la episiotomía, índice de Apgar mayor de 7 y algunos casos reportados de extracciones maternas y fetales. Consideraciones finales: Se concluye que la adopción de ANC en la red de atención para mujeres embarazadas es propicio para la atención materna y neonatal, sin excluir la atención hospitalaria para condiciones que requiere atención más compleja, así como el apoyo en situaciones de emergencia.Objetivo: Descrever os efeitos que a implementação dos Centros de Parto Normal (CPN) exerceu na qualidade da assistência obstétrica de acordo com os resultados maternos e neonatais. Métodos: Revisão de Literatura Integrativa, cujos artigos foram buscados nas bases de dados Scientific Eletronic Library Online (SciELO), Medical Literature Analysis and Retrievel System Online (Medline) e Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS). Resultados: Com pouco mais de duas décadas do início da implementação dos CPN’s no Brasil, estudos mostram impactos positivos desse sistema no cuidado às parturientes e aos neonatos. Nuliparidade sem comorbidades associadas, presença de acompanhantes e emprego de posições de parto não litotômicas estiveram presentes nos trabalhos analisados, influenciando os desfechos das parturientes de forma positiva: menor uso de ocitocina, diminuição da realização de episiotomias, índice Apgar maior que 7 e poucos casos relatados de remoções maternas e fetais.
