Nursing workload in intensive care unit: a mixed method study

Resumo: This study aimed to analyze the nursing workload of an Intensive Care Unit. It is a mixed method study with a convergent parallel design. Quantitative data were collected using the Nursing Activities Scores in 73 patient records and submitted to descriptive and correlational analysis. Qualitative data were collected through interviews with 18 nursing workers and analyzed using the content analysis. Later, the quantitative and qualitative data were analyzed together, aiming at their integration. It was verified that patients occupy 70% of the time of a nursing worker, which indicates a high nursing workload. This result was confirmed by qualitative data, organized into the following categories: Care demand of ICU patients at a university hospital and Work organization: repercussions on nursing workload and patient safety, that evidenced a perception of high workload and problems in the work organization that even interfere with patient safety. Quantitative and qualitative data converged with each other and indicated that nursing workers are overloaded.

Se trata de un estudio de método mixto, centrado en el tipo de diseño convergente paralelo. Los datos cuantitativos fueron recolectados a través del Nursing Activities Scores, en 73 registros de pacientes y sometidos a análisis descriptivo y de correlación. Los datos cualitativos fueron recolectados a través de entrevista con 18 trabajadores de enfermería y se analizaron mediante análisis de contenido. Posteriormente, los datos cuantitativos y cualitativos fueron analizados juntos, con el objetivo de integrarlos. Se verificó que los pacientes ocupan el 70% del tiempo de un trabajador de enfermería, lo que indica una alta carga de trabajo. Este resultado fue confirmado por los datos cualitativos, organizados en las siguientes categorías: Demanda de cuidados de pacientes de la UCI de un hospital universitario y Organización del trabajo: repercusiones en la carga de trabajo de enfermería y la seguridad del paciente, que evidenciaron una percepción de alta carga de trabajo y problemas en la organización del trabajo que incluso interfieren en la seguridad del paciente. Los datos cuantitativos y cualitativos convergieron entre sí y indicaron que los trabajadores de enfermería están sobrecargados.O presente estudo objetivou analisar a carga de trabalho da equipe de enfermagem de uma Unidade de Terapia Intensiva. Trata-se de um estudo de método misto, do tipo paralelo convergente. Os dados quantitativos foram coletados por meio do Nursing Activities Scores, em 73 prontuários de pacientes e submetidos a análise descritiva e correlacional. Os dados qualitativos foram coletados por meio de entrevista, com 18 trabalhadores de enfermagem e analisados via análise de conteúdo.  Após, os dados quantitativos e qualitativos, foram analisados conjuntamente, visando a integração entre os mesmos. Verificou-se que os pacientes ocupam em média 70% do tempo de um trabalhador de enfermagem, o que indica alta carga de trabalho.  Esse resultado foi confirmado por meio dos dados qualitativos, organizados nas seguintes categorias: Demanda de cuidados dos pacientes atendidos na UTI de um hospital universitário e Organização do trabalho: repercussões na carga de trabalho da enfermagem e na segurança do paciente, as quais evidenciaram uma percepção de elevada carga de trabalho e problemas na organização do trabalho que interferem, inclusive, na segurança do paciente.
