Characterization of pre kidney transplantation patients at a reference hospital in Fortaleza-CE

Resumo: | The previous nutritional status of individuals with end-stage renal disease has an important impact on the outcomes of Kidney Transplantation. Thus, the study aimed to characterize factors relevant to nutritional status in pre-kidney transplant patients at a referral hospital in Fortaleza-CE. Is characterized as a Cross-sectional, descriptive and retrospective study consisting of 136 patients aged over 18 years of both sexes admitted to undergo Kidney Transplantation from March 2018 to March 2020. Clinical, biochemical and anthropometric data were collected. Statistical analyzes were performed using Statistical Product and Service Solutions - SPSS® (version 20) software. Data were transmitted by Fisher's exact, chi-square (χ2), Kolmogorov-Smirnov, Student's t, Mann-Whitney tests. Significance level was set at p <0.05. There was an association between age and Body Mass Index (BMI). It was found that the elderly (≥ 60 years) had a higher odds ratio of having a BMI <18.5 kg / m² (p < 0.031). According to biochemical data, most of the sample has nutritional impairment according to Total Lymphocyte Count (TLC) and lymphocyte/monocyte ratio. It was found that patients aged ≥60 years of age had a lower mean lymphocyte/monocyte ratio when compared to patients aged <60 years. As for the comorbidities evaluated, Systemic Arterial Hypertension (SAH) was the most prevalent in the sample. Elderly people are more susceptible to nutritional impairment. Given the various factors that contribute to this condition, the clinical and nutritional assessment and monitoring of these patients is of great importance. |

El estado nutricional previo de las personas con enfermedad renal en etapa terminal tiene un impacto importante en los resultados del trasplante de riñón. Por lo tanto, el estudio tuvo como objetivo caracterizar los factores relevantes para el estado nutricional en pacientes en pre-trasplante renal en un hospital de referencia en Fortaleza-CE. Se caracteriza por ser un estudio transversal, descriptivo y retrospectivo conformado por 136 pacientes mayores de 18 años de ambos sexos ingresados para someterse a Trasplante Renal entre marzo de 2018 y marzo de 2020. Se recolectaron y analizaron datos clínicos, bioquímicos y antropométricos. Los análisis estadísticos se realizaron utilizando el software Statistical Product and Service Solutions - SPSS® (versión 20). Los datos se analizaron utilizando las pruebas exacta de Fisher, chi-cuadrado (χ2), Kolmogorov-Smirnov, t de Student, Mann-Whitney. Se fijó un nivel de significación en p < 0,05. Hubo una asociación significativa entre la edad y el índice de masa corporal (IMC). Se constató que los ancianos (≥ 60 años) tenían mayor razón de probabilidad de tener un IMC <18,5 kg/m² (p<0,031). Según los datos bioquímicos, la mayor parte de la muestra presenta deterioro nutricional según recuento total de linfocitos (CTL) y relación linfocitos/monocitos. Se encontró que los pacientes con edad ≥ 60 años tenían una proporción media de linfocitos/monocitos más baja en comparación con los pacientes con edad < 60 años. En cuanto a las comorbilidades evaluadas, la hipertensión arterial sistémica (HAS) fue la más prevalente en la muestra, siendo los ancianos los más susceptibles al deterioro nutricional. Ante varios factores que contribuyen a esta situación, la evaluación y seguimiento clínico y nutricional de estos pacientes es de gran importancia.O estado nutricional prévio dos indivíduos com doença renal crônica terminal possui importante impacto nos desfechos do Transplante Renal. Foram coletados e analisados dados clínicos, bioquímicos e antropométricos. As análises estatísticas foram realizadas utilizando o software Statistical Product and Service Solutions - SPSS® (versão 20). Os dados foram analisados pelos testes exato de Fisher, qui-quadrado (χ2), Kolmogorov-Smirnov, t de Student, Mann-Whitney. Foi considerado nível de significância fixado em p < 0,05. Houve associação significativa entre idade e índice de massa corpórea (IMC). Verificou-se que idosos (≥ 60 anos) tiveram maior razão de chance de apresentarem IMC <18,5 kg/m² (p<0,031). De acordo com os dados bioquímicos, maior parte da amostra, apresenta comprometimento nutricional de acordo com contagem total de linfócitos (CTL) e relação linfócito/monócito. Verificou-se que pacientes com idade ≥60 anos apresentaram menor média de relação linfócito/monócito quando comparados aos pacientes com idade <60 anos. Quanto às comorbidades avaliadas, a hipertensão arterial sistêmica (HAS) foi a mais prevalente na amostra. Idosos mostraram-se mais susceptíveis ao quadro de comprometimento nutricional. Diante de diversos fatores que contribuem para esse quadro é de grande importância a avaliação e o acompanhamento clínico e nutricional desses pacientes.
