Evaluation of droplet and aerosol dispersion: a biosafety alert for the dental surgeon in the context of the COVID-19 pandemic

Resumo: The pandemic of COVID-19 has called into question the risks of infection in dentistry. This research is justified by the need to demonstrate the occupational risks that droplets and aerosols pose to the dental surgeon. The study aims to analyze the dispersion of aerosol droplets in a dental environment and the effectiveness of PPE. We used simulations of a procedure in a dental office covered with white TNT, with the water in the reservoir stained with food coloring, and two models playing the role of the attendant and the patient. To keep the simulation closer to reality, two devices made of acrylic resin were superimposed on the patient's arch. The steps were photographed for data recording, analysis, and future comparisons. After the simulation, spills were found on the TNT that covered the floor, on the office table, and on the spittoon. The personal protective equipment used by the attendant was where the highest concentration of splashes was observed, especially on the disposable apron and the face shield. The ability to disperse droplets and aerosols was evidenced in this study. Droplets and aerosols represent a health risk for the dental surgeon. Biosafety care should be part of the daily routine of professionals.

Esta investigación se justifica por la necesidad de demostrar los riesgos laborales que las gotas y los aerosoles ofrecen al cirujano dental. El estudio pretende analizar la dispersión de las gotas de aerosol en un entorno dental y la eficacia del EPI. Se utilizaron simulaciones de un procedimiento en un consultorio dental cubierto con TNT blanco, con el agua del depósito coloreada con colorante alimentario y dos modelos haciendo el papel de asistente y paciente. Para que la simulación se acercara más a la realidad, se superpusieron dos dispositivos de resina acrílica en la arcada del paciente. Los pasos fueron fotografiados para el registro de datos, el análisis y las futuras comparaciones. Tras el simulacro, se encontraron salpicaduras en el TNT que cubría el suelo, en la mesa del despacho y en la escupidera. El equipo de protección individual utilizado por el operario fue donde se observó la mayor concentración de salpicaduras, especialmente en el delantal desechable y en la pantalla facial. La capacidad de dispersión de gotas y aerosoles se puso de manifiesto en el presente estudio. Las gotas y los aerosoles representan un riesgo para la salud del cirujano dental. El cuidado de la bioseguridad debe formar parte de la rutina diaria de los profesionales.A pandemia de COVID-19 pôs em questão os riscos de infecções existentes na odontologia. Esta pesquisa se justifica pela necessidade de demonstrar os riscos ocupacionais que as gotículas e aerossóis oferecem ao cirurgião-dentista. O estudo objetiva analisar a dispersão das gotículas aerossóis em ambiente odontológico e a eficácia dos EPIs. Foram usadas simulações de um procedimento em um consultório revestido por TNT branco, com a água do reservatório corada com corante alimentício e dois modelos fazendo o papel de atendente e paciente. Para manter a simulação mais próxima à realidade, dois dispositivos feitos de resina acrílica foram sobrepostos a arcada do paciente. As etapas foram fotografadas para registro de dados, análise e comparações futuras. Após a simulação, foram encontrados respingos no TNT que revestia o piso, na mesa do consultório e na cuspideira. No equipamento de proteção individual utilizado pelo atendente foi onde se observou a maior concentração de respingos, principalmente no avental descartável e no protetor facial. A capacidade de dispersão de gotículas e aerossóis foi evidenciada no presente estudo. As gotículas e aerossóis representam um risco para saúde do cirurgião-dentista.
