Risk factors for the development of acute kidney injury in patients hospitalized with COVID-19

Resumo: This study aims to analyze the predominant risk factors for the development of acute kidney injury in hospitalized patients with COVID-19. This is an integrative review of retrospective and prospective cohort studies and observational studies, with a search carried out in PubMed and Virtual Health Library databases with the keywords "SARS-CoV-2", "acute kidney injury", and "risk factors", selecting those that answered the guiding question “what are the predictors and risk factors for the development of acute kidney injury in hospitalized patients with COVID-19?”. Following this research and applying the year of publication and language filters, 39 articles were initially selected. After exclusions, 7 articles were included in the present review, which had their quality evaluated and their data collected using the PRISMA methodology. In all selected articles, the statistical significance of 11 risk factors was verified, such as high D-dimer, serum creatinine, C-reactive protein, increase in the number of leukocytes, decrease in lymphocytes, advanced age, gender, high body mass index, hypertension, diabetes and chronic kidney disease. After analysis of the selected articles, it was found that the most frequent risk factors for an AKI outcome are mainly advanced age, chronic kidney disease and elevated C-reactive protein on admission. Overall, these factors are risk indicators and require greater attention from professionals to provide an effective therapeutic approach and prevent serious outcomes.

Se trata de una revisión integradora de estudios de cohortes retrospectivos y prospectivos y estudios observacionales, con la búsqueda realizada en las bases de datos PubMed® y Biblioteca Virtual en Salud con las palabras clave “SARS-CoV-2”, “injuria renal aguda” y “factores de riesgo”, seleccionando aquellas que respondieron a la pregunta orientadora “¿cuáles son los predictores y factores de riesgo para el desarrollo de insuficiencia renal aguda en pacientes hospitalizados con COVID-19?”. Tras esta investigación y aplicando filtros de año de publicación e idioma, se seleccionaron inicialmente 39 artículos. Después de las exclusiones, 7 artículos fueron incluidos en la presente revisión, cuya calidad fue evaluada y sus datos recolectados utilizando la metodología PRISMA. En todos los artículos seleccionados, se verificó la significación estadística de los 11 factores de riesgo más frecuentes, como dímero D elevado, creatinina sérica, proteína C reactiva, aumento del número de leucocitos, disminución de linfocitos, edad avanzada, género, índice de masa corporal alto, hipertensión, diabetes y enfermedad renal crónica. Tras analizar los artículos seleccionados, se encontró que los factores de riesgo comunes más comunes para este desenlace de IRA son, principalmente, la edad avanzada, enfermedad renal crónica y proteína C reactiva elevada al ingreso. En general, estos factores son indicadores de riesgo y requieren una mayor atención por parte de los profesionales para proporcionar un enfoque terapéutico eficaz y prevenir resultados graves.Este estudo objetiva analisar os preditores e fatores de risco predominantes para desenvolvimento de lesão renal aguda (LRA) em pacientes hospitalizados com COVID-19. Trata-se de uma revisão integrativa de estudos coorte retrospectivos e prospectivos e de estudos observacionais, com a pesquisa realizada nas bases de dados PubMed® e Biblioteca Virtual em Saúde com as palavras-chave “SARS-CoV-2”, “acute kidney injury” e “risk factors”, selecionando os que respondessem a pergunta norteadora “quais são os preditores e fatores de risco para o desenvolvimento de lesão renal aguda em pacientes hospitalizados com COVID-19?”. Seguindo essa pesquisa e aplicando os filtros de ano de publicação e idioma, foram selecionados inicialmente 39 artigos. Após exclusões, 7 artigos foram incluídos na presente revisão, os quais tiveram sua qualidade avaliada e seus dados coletados com o uso das metodologia PRISMA. Em todos artigos selecionados, foi verificada a significância estatística de 11 fatores de risco mais frequentes, sendo elevado dímero D, creatinina sérica, proteína C reativa, aumento de número de leucócitos, diminuição de linfócitos, idade avançada, gênero, índice de massa corporal elevado, hipertensão, diabetes e doença renal crônica. Após análise dos artigos selecionados, constatou-se que os fatores de risco em comum mais recorrentes para tal desfecho de LRA são, principalmente, idade avançada, doença renal crônica e elevada proteína C reativa na admissão.
