Central Ossifying Fibroma in the maxilla: a rare case report

Resumo: Central ossifying fibroma is a slow-growing, circumscribed, benign and aggressive fibro-bony tumor, known for its expansive growth, the replacement of bone for a fibrous cellular tissue with a focus on mineralization. This lesion occurs in the third or fourth decades of life, with a predilection for females. It affects the mandibule in 70% to 80% of cases. Treatment includes enucleation and curettage or surgical resection for larger lesions. Thus, the aim of this article is to present and evaluate the resources of a clinical case of central ossifying fibroma in the maxilla. Female patient, 34 years old, HIV positive, appear on the service of maxillofacial surgery with painless swelling in the face for months after tooth extraction 12, resulting in feeding difficulties during chewing. On physical examination, diffuse intra and extra-oral edema is evident in the anterior region of the right maxilla, a hardened consistency with 10 months of growth. An incisional biopsy was performed and the material sent for histopathological analysis that concludes of benign bone lesions, suggestive of central ossifying fibroma. The patient underwent enucleation and surgical curettage, under general anesthesia, and was followed-up for 2 years, with no signs of recurrence.

Esta lesión generalmente ocurre en la tercera o cuarta décadas de la vida, con predilección por las mujeres. Afecta la mandíbula en 70% a 80% de los casos. El tratamiento incluye enucleación y legrado o resección quirúrgica para lesiones más grandes. Por lo tanto, el objetivo de este artículo es presentar y evaluar las características de un caso clínico de fibroma osificante central en el maxilar superior. Paciente femenina, de 34 años, VIH positiva, asistió al servicio de cirugía maxilofacial quejándose de hinchazón indolora en la cara durante 12 meses después de la extracción del diente 12, lo que resulta en dificultades para comer durante la masticación. El examen físico reveló edema intraoral y extraoral difuso en la región anterior del maxilar derecho, de consistencia endurecida, con 10 meses de crecimiento. Se realizó una biopsia incisional y el material se envió para un análisis histopatológico concluyente de la lesión ósea benigna, lo que sugiere un fibroma osificante central. El paciente fue sometido a enucleación y legrado quirúrgico, bajo anestesia general, seguido 2 años después de la operación, sin signos de recurrencia.O fibroma ossificante central é um tumor fibro-ósseo de crescimento lento bem circunscrito, raro, benigno e agressivo. Tal lesão também é conhecida por aumentar de forma expansiva e é caracterizada pela substituição de osso por um tecido celular fibroso contendo focos de mineralização. Essa lesão geralmente ocorre no terceiro ou quarta décadas de vida com uma predileção feminina. Afeta a mandíbula em 70% a 80% dos casos. O tratamento compreende enucleação e curetagem ou ressecção cirúrgica para lesões maiores. Assim, o objetivo deste artigo é apresentar e avaliar as características de um caso clínico de um paciente com Fibroma Ossificante Central em maxila.  Foi descrito um caso de uma paciente do sexo feminino, 34 anos de idade, HIV positivo, cuja queixa era de aumento de volume indolor em rosto há meses após exodontia do dente 12, resultando em dificuldades de se alimentar durante a mastigação. O exame físico evidenciou-se uma edema difuso intra e extra-oral em região anterior de maxila direita, endurecido à palpação, que foi submetida a enucleação mais curetagem cirúrgica. Foi feita uma biópsia incisional e levado material para análise no laboratório de anatomia histopatológica de uma universidade, no qual se obteve o resultado de lesão óssea benigna, sugestiva de fibroma ossificante central.
