Psamomatoid juvenile ossifying fibroma: case report

Resumo: Fibro-osseous lesions are a group of lesions characterized by the replacement of normal bone with fibrous animal tissue and mineralized tissue. Among these lesions is that the juvenile ossifying fibroma, a rare benign fibro-osseous tumor, comprising approximately 2% of oral tumors affecting children and adolescents. consistent with its histopathological features, it's subdivided into psammomatoid juvenile ossifying fibroma and trabecular juvenile ossifying fibroma. The psammomatoid variant is male, with a mean age of twenty-two years, and predominantly within the maxilla. this sort of tumor is typically discovered during a routine radiographic examination, and its cortical expansion may result during a detectable facial enlargement. Its treatment and prognosis are uncertain. For smaller lesions local excision or careful curettage are often performed. For rapidly growing and/or aggressive lesions, in masse resection with conservative margins associated or not with the utilization of bone grafts and reconstruction plates could also be necessary. The aim of this report is to explain the condition of a 26-year-old patient referred to the Hospital Universitário do Oeste do Paraná with a diagnosis of psammomatoid juvenile ossifying fibroma within the mandible. After clinical examination and analysis of imaging exams, endodontic treatment of elements 34 to 42 was performed. The treatment consisted of curettage, enucleation of the lesion and apicectomy of the teeth involved, evolving uneventfully postoperatively. The patient has been under follow-up for 01 year and 07 months.

Por medio de esas lesiones está el fibroma osificante juvenil, un tumor fibro-óseo benigno, raro, alcanzando aproximadamente 2% de los tumores bucales que acometen niños y adolescentes. De acuerdo con las características histopatológicas, es subdividido en fibroma osificante juvenil psamomatoide y fibroma osificante juvenil trabecular. La variante psamomatoide presenta predilección por el género masculino, edad media de 22 años, con predominancia en el maxilar. Generalmente ese tipo de tumo es descubierto en examen radiográfico de rutina, siendo que su expansión cortical puede resultar en un aumento fácil detectable. Su tratamiento y pronóstico son inciertos. Para las sesiones menores puede ser realizada la extirpación local o un legrado cuidadoso. Para las lesiones de crecimiento rápido y/o agresivo puede ser necesaria una resección en bloco con márgenes conservadoras asociada o no al uso de injerto óseo y placas de reconstrucción. El objetivo del informe es describir la condición de una paciente de 26 años, encaminada al Hospital Universitario del Oeste de Paraná con diagnóstico de fibroma osificante juvenil psamomatoide en mandíbula. Después de la realización del examen clínico y análisis de los exámenes de imagen, fue realizado el tratamiento endodóntico de los elementos 34 a 42. El tratamiento compuso en el legrado, enucleación de la lesión y apicectomía de los dientes involucrados, evolucionando con un postoperatorio sin intercurrencias, La paciente está en proservación hace 01 año y 07 meses.As lesões fibro-ósseas são um grupo de lesões caracterizadas pela substituição do osso normal por um tecido conjuntivo fibroso e tecido mineralizado. Dentre essas lesões está o fibroma ossificante juvenil, um tumor fibro-ósseo benigno, raro, abrangendo aproximadamente 2% dos tumores bucais que acometem crianças e adolescentes. De acordo com as características histopatológicas, é subdividido em fibroma ossificante juvenil psamomatoide e fibroma ossificante juvenil trabecular. A variante psamomatoide apresenta predileção pelo gênero masculino, idade média de 22 anos, com predominância na maxila. Geralmente esse tipo de tumor é descoberto em exame radiográfico de rotina, sendo que sua expansão cortical pode resultar em um aumento facial detectável. Seu tratamento e prognóstico são incertos. Para as lesões menores pode ser realizada a excisão local ou uma curetagem cuidadosa. Para as lesões de crescimento rápido e/ou agressivo pode ser necessária uma ressecção em bloco com margens conservadoras associada ou não ao uso de enxertos ósseos e placas de reconstrução. O objetivo do relato é descrever a condição de uma paciente de 26 anos, encaminhada ao Hospital Universitário do Oeste do Paraná com diagnóstico de fibroma ossificante juvenil psamomatoide em mandíbula. Após a realização do exame clínico e análise dos exames de imagem, foi realizado o tratamento endodôntico dos elementos 34 ao 42. O tratamento consistiu na curetagem, enucleação da lesão e apicectomia dos dentes envolvidos, evoluindo com um pós-operatório sem intercorrências.
