Direct ophthalmoscopy skills among Medical students

Resumo: Direct ophthalmoscopy is part of physical examinations applied in patients; it is relevant for the diagnosis and prognosis of several diseases, since several systemic diseases lead to changes in the posterior segment of the eye. The aim of the present research is to investigate the ability of graduates in Medicine in recognizing changes in normal eye fundus deriving from illnesses that affect the human body. The adopted research methodology was based on direct ophthalmoscopy workshops carried out with 49 students; questionnaires with multiple choices alternatives about 8 images of funduscopic manifestations, 1 normal retinography and 7 images with systemic manifestations of prevalent diseases were applied. Results have shown little familiarity of some participants with the identification and differentiation of eye fundus normality based on the approached diseases. However, it was possible observing that participant students did not have much knowledge about normal eye fundus. Thus, they had difficulty in distinguishing different patterns of fundus outcomes compatible to their respective diseases. Nevertheless, they presented good knowledge on what is expected to find in normal eye fundus.

La oftalmoscopia directa forma parte del examen físico del paciente y es relevante para el diagnóstico y pronóstico de diversas enfermedades, ya que muchas enfermedades sistémicas conducen a alteraciones en el segment posterior del ojo. El objetivo de esta investigación es investigar la habilidad de los estudiantes de medicina para reconocer alteraciones del fondo de ojo normal, resultantes de enfermedades que afectan al cuerpo humano. La metodología de investigación adoptada se basó en talleres directos de oftalmoscopia con un grupo de 49 estudiantes; se aplicaron cuestionarios que contenían alternativas de opción múltiple que incluyeron 8 imágenes de manifestaciones fundoscópicas, 1 retinografía normal y 7 imágenes con manifestaciones sistémicas de enfermedades prevalentes. Los resultados demostraron la poca familiaridad de parte de los participantes con la identificación y diferenciación de la anormalidad del fondo de ojo según las enfermedades abordadas. Sin embargo, fue posible observar que los estudiantes que participaron en el estudio no tenían conocimientos sobre el fondo de ojo normal. Por lo tanto, los estudiantes de medicina tienen dificultades para distinguir los diferentes patrones de hallazgos fundoscópicos compatibles con sus respectivas enfermedades. Sin embargo, tienen un dominio de lo que se espera que se encuentre en el fondo de ojo normal.Oftalmoscopia direta é parte do exame físico do paciente e é relevante para o diagnóstico e prognóstico de diversas doenças, haja vista que muitas doenças sistêmicas levam a alterações no segmento posterior do olho. O objetivo da presente pesquisa é investigar a habilidade de graduandos em Medicina em reconhecer alterações do fundo de olho normal, decorrente de enfermidades que afetam o corpo humano. A metodologia da pesquisa adotada baseou-se na realização de oficinas de oftalmoscopia direta com um grupo de 49 acadêmicos; foram aplicados questionários contendo alternativas de múltipla escolha que abrangiam 8 imagens de manifestações fundoscópicas, 1 retinografia normal e 7 imagens com manifestações sistêmicas de doenças prevalentes. Os resultados demonstraram a pouca familiaridade de parte dos participantes com a identificação e diferenciação de anormalidade do fundo de olho de acordo com as doenças abordadas. Contudo, foi possível observar que os acadêmicos participantes do estudo não possuíam conhecimento sobre fundo de olho normal. Logo, formandos em Medicina têm dificuldade em distinguir os diferentes padrões de achados fundoscópicos compatíveis com suas respectivas doenças. No entanto, apresentam domínio sobre o que se espera encontrar no fundo de olho normal.
