Heart failure associated with anemia: Use of intravenous iron compared to placebo

Resumo: Heart failure (HF) is a syndrome in which the heart has its capacity to fill or eject the ventricles decreased. Iron deficiency is commonly associated with HF and has been shown to be a factor of worse prognosis. The aim of this study is to evaluate patients affected by heart failure for intravenous iron supplementation compared to non-supplementation (or placebo). In this systematic review, a bibliographic search was conducted in the electronic databases Virtual Health Library, Capes Journal Portal and SciSearch. The Health Sciences Descriptors (DeCS) Heart Failure, Iron deficiency anemia and Iron Deficiency were used. The GRADE System was used to evaluate the quality of evidence of the articles analyzed, and 11 Randomized Clinical Studies covering 5 nationalities were included. Symptomatic patients diagnosed with HF associated with Ironpriva Anemia showed improvements in primary outcomes on the global self-assessment scale and in the New York Heart Association (NYHA) functional class when receiving intravenous iron replacement when compared to those receiving placebo. In addition, there were no significant improvements in patients regarding oral iron treatment. Thus, it is concluded that anemia has important repercussions for patients affected by HF, being related to a worse prognosis. Therefore, intravenous iron supplementation is necessary to improve symptoms and quality of life, and should be monitored by specialists due to the possible adverse reactions that may appear.

La deficiencia de hierro se asocia comúnmente con la insuficiencia cardíaca y se ha demostrado que es un factor de peor pronóstico. El objetivo de este estudio es evaluar a los pacientes que padecen insuficiencia cardíaca con respecto a la suplementación de hierro por vía intravenosa en comparación con la ausencia de suplementación (o placebo). En esta revisión sistemática se realizó una búsqueda bibliográfica en las bases de datos electrónicas Virtual Health Library, Capes Journal Portal y SciSearch. Se utilizaron los descriptores de ciencias de la salud (DeCS) insuficiencia cardíaca, anemia ferropénica y ferropenia. Se utilizó el Sistema GRADE para evaluar la calidad de la evidencia de los artículos analizados, incluidos 11 Estudios Clínicos Aleatorizados que cubren 5 nacionalidades. Los pacientes sintomáticos diagnosticados con IC asociada con anemia por deficiencia de hierro mostraron mejoras en los resultados primarios en la escala de autoevaluación global y la clase funcional de la New York Heart Association (NYHA) cuando recibieron reemplazo de hierro por vía intravenosa en comparación con los que recibieron placebo. Además, no hubo mejoras significativas en los pacientes con tratamiento con hierro oral. Así, se concluye que la anemia tiene importantes repercusiones para los pacientes afectados de IC, al estar relacionada con un peor pronóstico. Por tanto, la suplementación con hierro por vía intravenosa es necesaria para mejorar la sintomatología y la calidad de vida, debiendo ser monitorizada por especialistas ante las posibles reacciones adversas que puedan aparecer.A insuficiência cardíaca (IC) consiste em uma síndrome em que o coração apresenta sua capacidade de enchimento ou ejeção dos ventrículos diminuída. A deficiência de ferro está comumente associada a IC e vem sendo evidenciada como um fator de pior prognóstico. O objetivo desse estudo consiste em avaliar os pacientes acometidos por insuficiência cardíaca quanto à suplementação de ferro endovenoso em comparação com a não suplementação (ou placebo). Nesta revisão sistemática, realizou-se uma busca bibliográfica nas bases de dados eletrônicas Biblioteca Virtual em Saúde, Portal de Periódicos Capes e SciSearch. Foram utilizados os Descritores em Ciências da Saúde (DeCS) Insuficiência Cardíaca, Anemia ferropriva e Deficiência de Ferro. O Sistema GRADE foi utilizado para avaliar a qualidade de evidência dos artigos analisados, tendo sido incluído 11 Estudos Clínicos Randomizados que abrangem 5 nacionalidades. Os pacientes sintomáticos e diagnosticados com IC associada à Anemia Ferropriva apresentaram melhoras nos desfechos primários na escala da auto avaliação global e na classe funcional da New York Heart Association (NYHA) ao receberem reposição de ferro endovenoso quando comparado aos que receberam placebo. Além disso, não houve melhoras significativas nos pacientes quanto ao tratamento por ferro oral. Assim, conclui-se que a anemia tem repercussões importantes aos pacientes acometidos a IC, estando relacionada a um pior prognóstico.
