A look at the health care network: organization and challenges of the Network of Care for Persons with Disabilities

Resumo: This study aimed to present, discuss and reflect on the formation of the Network of Care for Persons with Disabilities (RCPD) and its challenges. An Integrative Review was carried out, selecting articles published in the LILACS, SciELO, Pub Med and Google Books databases. 11 publications from public institutions and 25 articles were included in the final sample. The data were analyzed and systematized in 3 segments: The Health Care Network: constitutive and organizational elements; The RCPD and the proposals for action, challenges and weaknesses. Factors such as the permanent training of health professionals, the integration with Higher Education Institutions and the organization of services are necessary so that through the subjectivity of the humanized embracement the transformation of actions into impacting factors in the daily life and in the production of the health process of these individuals occurs. In spite of the advances assured by the legislation, the daily life of the health services does not reflect these rights yet. The fragmentation of the network of assistance to patients with disabilities has not provided the expected resolute and equitable assistance, either because of the lack of training of the health team, or the low management power of the services and even the low funding provided for this area. It is evident the need to expand and qualify the access to health care for this vulnerable population, based on the resoluteness implemented in the assistance network.

Se realizó una revisión integradora, seleccionando artículos publicados en las bases de datos LILACS, SciELO, Pub Med y Google Books. En la muestra final se incluyeron 11 publicaciones de instituciones públicas y 25 artículos. Los datos se analizaron y sistematizaron en 3 segmentos: La Red Sanitaria: elementos constitutivos y organizativos; El RCPD y las propuestas de actuación, retos y debilidades. Factores como la formación permanente de los profesionales de la salud, la integración con las Instituciones de Educación Superior y la organización de los servicios son necesarios para que, a través de la subjetividad del abrazo humanizado, se produzca la transformación de las acciones en factores de impacto en la vida cotidiana y en la producción del proceso de salud de estos individuos. A pesar de los avances garantizados por la legislación, el día a día de los servicios sanitarios aún no refleja estos derechos. La fragmentación de la red de asistencia a los pacientes con discapacidad no ha proporcionado la esperada asistencia resolutiva y equitativa, ya sea por la falta de formación del equipo sanitario, ya sea por el escaso poder de gestión de los servicios e incluso por la escasa financiación prevista para este ámbito. Es evidente la necesidad de ampliar y cualificar el acceso a la atención sanitaria de esta población vulnerable, a partir de la resolutividad implementada en la red asistencial.Este estudo teve como objetivo apresentar, discutir e refletir sobre a formação da Rede de Cuidado à Pessoa com Deficiência (RCPD) e seus desafios. Foi realizada uma Revisão Integrativa, elegendo-se artigos publicados nas bases de dados da LILACS, do SciELO, do Pub Med e do Google Books. Foram incluídas 11 publicações de Instituições Públicas e 25 artigos na amostra final. Os dados foram analisados e sistematizados em 3 segmentos: a Rede de Assistência à Saúde: elementos constitutivos e organizacionais; a RCPD e as propostas de ação, desafios e fragilidades. Fatores como a capacitação permanente dos profissionais de saúde, a integração com as Instituições de Educação Superior e a organização dos serviços são necessárias para que através da subjetividade do acolhimento humanizado ocorra a transformação das ações em fatores impactantes no cotidiano da vida e na produção do processo de saúde desses indivíduos. Em que pese os avanços assegurados pela legislação, o cotidiano dos serviços de saúde ainda não reflete esses diretos. A fragmentação da rede de assistência ao paciente com deficiência não tem proporcionado a assistência resolutiva e equânime prevista, quer por falta de capacitação da equipe de saúde, quer por baixo poder de gestão dos serviços e até o baixo financiamento previsto para essa área.
