Trend and epidemiological profile of congenital anomalies in newborns in São Paulo (2000 to 2019)

Resumo: The aim of this study was to describe the trend and profile of epidemiological congenital anomalies (CA) in newborns in the State of São Paulo, from 2000 to 2019 and to analyze possible factors associated with the disease. Ecological time series study, using data from Live Births Information System (SINASC). Congenital anomalies and Live births number were extracted from SINASC to calculate incidence. Linear regression models were estimated for number of births with anomalies and for incidence of anomalies, on the basis of year. To analyze the associations of CA with the characteristics of the mother, prenatal/delivery and newborn, the chi-square test was used. Test significance level of 5%. In the analyzed period 12.364.643 births occurred in the state of São Paulo, of which 119.746 showed congenital anomalies (incidence of 0.97%, or 9.7 cases per 1,000 live births). A significant increase in the incidence of congenital anomalies (per 1,000 live births) was observed in the State of São Paulo in 2000 (5.72) and 2019 (12.54). There was an association of CA with maternal variables (age ≥40 years, schooling ≥12 years, mothers without a partner). Related to prenatal care/delivery (none and 1-3 prenatal consultations, duration of preterm pregnancy < 37 weeks, triple deliveries or more and operative); newborns (male, black, brown and yellow race/color, low birth weight <2,500g, and Apgar score from 0 to 3 in the 1st-5th). It is necessary to know the tendency and distribution of congenital anomalies for better targeting of Public Policies related to the binomial.

Estudio ecológico de serie temporal, utiliza datos del Sistema de información de Nacidos Vivos (SINAC). Inicialmente fueron extraídos del SINAC las AC y el número de nacidos vivos para cálculo de incidencia, luego fueron estimados modelos de regresión linear para los nacidos con anomalías e incidencias de anomalía, en el año. Para análisis de las asociaciones de las AC con las características de la madre, de prenatal/parto y del recién nacido, fue utilizado el test ji-cuadrado. Nivel de significación de los testeos de un 5%. En el período analizado hubo 12.364,643 nacimientos en el estado; 119.746 presentaban AC e incidencia de 0,97%, o 9,7 casos por 1.000 nacidos vivos. En cada 1.000 se observó aumento significativo de AC, en 2000 (5,72 y 2019 (12,54); asociación de las AC con variables maternas (edad ≥ 40 años, escolaridad ≥ 12 años, madres sin compañero); relacionadas al prenatal/parto (ninguna y de 1-3 consultas de prenatal, duración de gestación pretérmino < 37 semanas, partos triples o más y operativos); recién nacidos (sexo masculino, raza/color negro, pardo y amarilla, bajo peso al nacer <2.500g, y Apgar de 0 a 3 en el 1º- 5º minutos). Es necesario conocer la propensión y distribución de las AC para mejor direccionamiento de las Políticas Públicas relacionadas al binomio.O objetivo deste estudo foi descrever a tendência e o perfil epidemiológico das anomalias congênitas (AC) em recém-nascidos no Estado de São Paulo, de 2000 a 2019 e analisar possíveis fatores associados ao agravo. No período analisado ocorreram 12.364.643 nascimentos no estado e destes 119.746 apresentavam anomalias congênitas (incidência de 0,97%, ou 9,7 casos por 1.000 nascidos vivos). Observou-se aumento significativo da incidência (em 1.000 nascidos vivos) de anomalias congênitas no Estado de São Paulo, em 2000 (5,72) e 2019 (12,54). Houve associação das AC com variáveis maternas (idade ≥40 anos, escolaridade ≥12 anos, mães sem companheiro); relacionadas ao pré-natal/parto (nenhuma e de 1-3 consultas de pré-natal, duração da gestação pré-termo < 37 semanas, partos triplos ou mais e operatórios); e recém-nascidos (sexo masculino, raça/cor preta, parda e amarela, baixo peso ao nascer <2.500g, e Apgar de 0 a 3 no 1º- 5º minutos).
