Cenário do acesso ao tratamento fisioterapêutico de indivíduos acometidos por Acidente Vascular Cerebral: Uma revisão integrativa

Resumo: The objective of this review was to identify in the literature the factors that limit the access of post-stroke individuals to physiotherapeutic treatment in health systems. The search was carried out in the following databases: PubMed, Scopus, BVS, Cochrane, Embase, Pedro and Scielo and, in order to complement the search, gray literature (Google Academic) was used. The following search terms were used: “strokes”, “therapeutic”, “publichealth”, “accesshealthservices”, “physiotherapy” present in Mesh and DeCS. The search included articles from the last 6 years, in Portuguese, English and Spanish. 1991 articles were found, after applying the criteria for inclusion and elimination of duplicates, 13 articles were analyzed, in which 10 answered the research question. The analyzed studies pointed out difficulties in accessing physiotherapeutic rehabilitation, the barriers that prevent admission were similar, being lack of referral, difficulty in moving to rehabilitation centers, unavailability of vacancy, bureaucracy in scheduling and long waiting time. In general, it was seen that there are still difficulties to get treatment, in which, according to the studies, access to physiotherapeutic treatment was seen as difficult to reach and insufficient, being related to financial, physical and bureaucratic issues. In this way, the studies carried out allowed us to know the status of post-stroke patients' access to physiotherapeutic care, which enables the development of strategies aimed at improving accessibility to rehabilitation services.

La búsqueda se realizó en las siguientes bases de datos: PubMed, Scopus, BVS, Cochrane, Embase, Pedro y Scielo y, para complementar la búsqueda, se utilizó literatura gris (Google Academic). Se utilizaron los siguientes términos de búsqueda: “strokes”, “therapeutic”, “publichealth”, “accesshealthservices”, “fisioterapia” presentes en Mesh y DeCS. La búsqueda incluyó artículos de los últimos 6 años, en portugués, inglés y español. Se encontraron 1991 artículos, luego de aplicar los criterios de inclusión y eliminación de duplicados, se analizaron 13 artículos, en los cuales 10 respondieron la pregunta de investigación. Los estudios analizados señalaron dificultades en el acceso a la rehabilitación fisioterapéutica, las barreras que impiden el ingreso fueron similares, siendo la falta de derivación, la dificultad para trasladarse a los centros de rehabilitación, la indisponibilidad de vacantes, la burocracia en la programación y el largo tiempo de espera. En general, se vio que aún existen dificultades para acceder al tratamiento, en los cuales, según los estudios, el acceso al tratamiento fisioterapéutico fue vistocomo difícil e insuficiente, estando relacionado con cuestiones financieras, físicas y burocráticas. Así, los estudios realizados permitieron conocer el estado de acceso de los pacientes postictus a la atención fisioterapéutica, lo que posibilita el desarrollo de estrategias encaminadas a mejorar la accesibilidad a los servicios de rehabilitación.O objetivo desta revisão foi identificar na literatura os fatores que limitam o acesso de indivíduos pós AVC ao tratamento fisioterapêutico nos sistemas de saúde. A busca foi realizada nas bases de dados: PubMed, Scopus, BVS, Cochrane, Embase, Pedro e Scielo e com intuito de complementar a busca foi utilizada a literatura cinzenta (Google Acadêmico). Foram usados os seguintes termos de busca: “strokes”, “therapeutic”, "publichealth", "accesshealthservices", “physiotherapy” presentes no Mesh e DeCS. A busca incluiu artigos dos últimos 6 anos, nas línguas portuguesa, inglesa e espanhola. Foram encontrados 1991 artigos, após a aplicação dos critérios de inclusão e eliminação de duplicatas foram analisados 13 artigos,no qual 10 responderam a questão da pesquisa. Os estudos analisados apontaram dificuldades no acesso à reabilitação fisioterapêutica, as barreiras que impedem a admissão foram semelhantes, sendo falta de encaminhamento, dificuldade de deslocamento para os Centros de reabilitação, indisponibilidade de vaga, burocracias no agendamento e longo tempo de espera. De maneira geral, foi visto que aindahá dificuldades para conseguir tratamento, no qual, de acordo com os estudos, o acesso ao tratamento fisioterapêutico foi visto de difícil alcance e insuficiente, estando relacionado a questões financeiras, físicas e burocráticas.
