Surgical management after perforation of Schneider's membrane during sinus lift: case report

Resumo: Treatment with dental implants has been used to rehabilitate toothless patients through the fixed prosthesis on implants. In the maxilla, the alveolar bone may be compromised by bone resorption and pneumatization of the maxillary sinus, resulting in insufficient bone height for the implant installation. Thus, the lifting of the maxillary sinus (sinus lift) is a technique that allows the gain of bone volume in the posterior region of the maxilla, enabling rehabilitation with implants. This study aims to highlight a case where Schneider's membrane was perforated during a sinus lift and resolved during the operation. A 63-year-old male patient sought dental care complaining about his removable dental prostheses and longing to replace them with fixed prostheses on implants. On clinical and imaging examination, pneumatization of the maxillary sinus and insufficient bone height were found to install implants in the posterior region of the maxilla. The patient underwent a right maxillary sinus lift and during the procedure there was a 6mm perforation of the Schneidarian Membrane. This complication was solved by suturing the membrane to a perforation in the lateral bone wall of the maxillary sinus, which allowed the procedure to continue and be successful. It is concluded that the perforation of the Schneider membrane during the sinus lift procedure is a common trans-surgical complication and depending on the size of the perforation, there is a more appropriate approach to be performed. The technique chosen by the professional should be based on his clinical experience and scientific evidence.

En el maxilar, el hueso alveolar puede verse comprometido por la resorción ósea y la neumatización del seno maxilar, lo que da como resultado una altura ósea insuficiente para la instalación del implante. Así, la elevación del seno maxilar (sinus lift) es una técnica que permite ganar volumen óseo en la región posterior del maxilar, posibilitando la rehabilitación con implantes. Este estudio tiene como objetivo destacar un caso en el que la membrana de Schneider se perforó durante un levantamiento de seno y se resolvió durante la operación. Un paciente masculino de 63 años buscó atención dental quejándose de sus prótesis dentales removibles y anhelando reemplazarlas con prótesis fijas sobre implantes. En el examen clínico y de imagen, se encontró neumatización del seno maxilar y altura ósea insuficiente para instalar implantes en la región posterior del maxilar. El paciente fue sometido a una elevación del seno maxilar derecho y durante el procedimiento hubo una perforación de 6 mm de la Membrana Schneidariana. Esta complicación se solucionó suturando la membrana a una perforación en la pared ósea lateral del seno maxilar, lo que permitió que el procedimiento continuara y fuera exitoso. Se concluye que la perforación de la membrana de Schneider durante el procedimiento de elevación del seno es una complicación transquirúrgica común y, dependiendo del tamaño de la perforación, existe un abordaje más apropiado a realizar. La técnica elegida por el profesional debe basarse en su experiencia clínica y evidencia científica.O tratamento com implantes dentários vem sendo usado para reabilitar os pacientes desdentados através da prótese fixa sobre implantes. Dessa forma, o levantamento do seio maxilar (sinus lift) é uma técnica que permite o ganho de volume ósseo na região posterior da maxila, possibilitando a reabilitação com implantes. Esse estudo tem como objetivo evidenciar um caso onde houve perfuração da membrana de Schneider durante um sinus lift e a sua resolução no transoperatório. Um paciente do sexo masculino, 63 anos, buscou atendimento odontológico queixando-se de suas próteses dentárias removíveis e ansiando substituí-las por próteses fixas sobre implantes. Ao exame clínico e de imagem, foi constatada a pneumatização do seio maxilar e altura óssea insuficiente para instalação de implantes na região posterior de maxila. O paciente foi submetido ao levantamento do seio maxilar direito e durante o procedimento houve uma perfuração da Membrana Schneidariana equivalente a 6mm. Essa complicação foi solucionada através da sutura da membrana a uma perfuração na parede óssea lateral do seio maxilar, o que permitiu a continuidade e sucesso do procedimento. Conclui-se que a perfuração da membrana de Schneider durante o procedimento sinus lift é uma intercorrência transcirúrgica comum e dependendo do tamanho da perfuração há uma conduta mais adequada a ser realizada.
