Assistance to twins at risk of congenital COVID-19 infection, experience report

Resumo: This study aimed to describe the assistance and characterize the clinical evolution of premature twins at risk of congenital infection, born to a mother infected with SARS-CoV-2 and with a clinical picture of COVID-19. It is an experience report described from the medical records of the twins and their mother. Premature twins, gestational age 28 weeks and three days, both male. The twin I weighed 1050 grams and the second, 698 grams. At 36h of life, collection was performed by means of swab of oro and nasopharynx, of both, for RT-PCR research for SARS-CoV-2, positive in both. Twin I died after 48 hours of life and II stayed in the Neonatal Intensive Care Unit for more than 2 months. There was no contact of the mother with the twins at the moment of birth until the fifteenth day of life of the twin II, there was also no breastfeeding during this period. New RT-PCR research for SARS-CoV-2 and rapid test performed on the fifteenth day of life were negative. The possibility of mother-to-child transmission and the consequences that this virus can cause on the fetus is worrying. However, there are several reports that suggest a low potential for this transmission, yet this possibility cannot be ruled out and must be ensured by adequate assistance with trained staff and all transmission care taken after birth.

Este es un relato de experiencia descrito a partir de los registros médicos de los gemelos y su madre. Gemelos prematuros, edad gestacional 28 semanas y tres días, ambos varones. El gemelo I pesaba 1050 gramos y el segundo 698 gramos. A las 36 h de vida, se realizó la recolección mediante swab de oro y nasofaringe, de ambos, para la investigación por RT-PCR para SARS-CoV-2, positivo en ambos. Gemelo I falleció a las 48 horas de vida y II permanecí en la Unidad de Cuidados Intensivos Neonatales por más de 2 meses. No hubo contacto de la madre con los gemelos al momento del nacimiento hasta el día quince de vida del gemelar II, tampoco hubo amamantamiento durante este período. La nueva investigación de RT-PCR para el SARS-CoV-2 y la prueba rápida realizada en el decimoquinto día de vida presentaron resultados negativos. Es preocupante la posibilidad de transmisión de madre a hijo y las consecuencias que este virus puede causar en el feto. Sin embargo, hay varios informes que sugieren un bajo potencial de transmisión, todavía esta posibilidad no se puede descartar y debe garantizarse mediante la asistencia adecuada con personal capacitado y todos los cuidados de transmisión después del nacimiento.Este estudo teve por objetivo descrever a assistência e caracterizar a evolução clínica de gemelares prematuros com risco de infecção congênita, nascidos de mãe infectada pelo SARS-CoV-2 e com quadro clínico de COVID-19. Trata-se de um relato de experiência descrito a partir de registros dos prontuários dos gemelares e de sua genitora. Gemelares prematuros, idade gestacional 28 semanas e três dias, ambos do sexo masculino. O gemelar I pesou 1050 gramas e o II, 698 gramas. Às 36h de vida foi realizada coleta por meio de swab de oro e nasofaringe, de ambos, para pesquisa de RT-PCR para SARS-CoV-2, positiva nos dois. O gemelar I faleceu com 48h de vida e o II ficou interno por mais de 2 meses na Unidade de Terapia Intensiva Neonatal. Não houve contato da genitora com os gemelares no momento do nascimento até o décimo quinto dia de vida do gemelar II, também não houve aleitamento materno nesse período. Nova pesquisa de RT-PCR para SARS-CoV-2 e teste rápido realizado no décimo quinto dia de vida foram negativos. É preocupante a possibilidade de transmissão de mãe para filho e as consequências que esse vírus pode causar no feto.
