Resumo: Unna boot therapy requires from the nurse an intense commitment related to technical and scientific knowledge, in order to eradicate possible complications. This study aims to report the experience of the Healing Program in the face of care for people with venous ulcers and the iatrogenic consequences resulting from inadequate handling of Unna boot. The methodology applied refers to an experience report, with a descriptive and qualitative approach, developed by a research and extension program of the undergraduate nursing course at the Federal University of São João Del Rei - Campus Centro Oeste, which provides care to people with skin lesions in the University Stomatherapy clinic. In 2021, the Cicatriza team came across the case of a patient with venous insufficiency, presenting an iatrogenic lesion characterized by the exposure of tendons, in the right lower limb (MID). Compressive therapy is considered the main alternative for the treatment of venous ulcers, however, not applied correctly, the harm outweighs the benefits, causing an ineffective treatment and increasing recurrence rates, the cost of treatment, the amount of exudate and consequently the risks of severe complications. Therefore, given the experience of the case, it is evident that the success in the treatment of venous ulcers is complex, dynamic and multifactorial. It is up to the nurse to manage the clinic, which involves the indication of therapies, appropriate products, professional evaluation and guidance, and therapeutic consent.

El objetivo de este estudio es dar a conocer la experiencia del Programa Cura en el cuidado de personas con úlceras venosas y los efectos iatrogénicos derivados del manejo inadecuado de la bota de Unna. La metodología aplicada se refiere a un relato de experiencia, con un enfoque descriptivo y cualitativo, desarrollado por un programa de investigación y extensión del curso de pregrado en enfermería de la Universidad Federal de São João Del Rei - Campus Centro Oeste, que atiende a personas con lesiones cutáneas en la clínica ambulatoria de Estomatoterapia de la Universidad. En 2021, el equipo de Cicatriza se enteró del caso de un paciente con insuficiencia venosa, que presentaba una lesión iatrogénica caracterizada por exposición del tendón, en el miembro inferior derecho (MIR). La terapia compresiva se considera la principal alternativa para el tratamiento de las úlceras venosas, sin embargo, si no se aplica correctamente, los daños superan a los beneficios, lo que conduce a un tratamiento ineficaz y aumenta las tasas de recaída, el costo del tratamiento, la cantidad de exudado y, por lo tanto, el riesgo de complicaciones graves. Por tanto, dada la experiencia del caso, es evidente que el éxito en el tratamiento de las úlceras venosas es complejo, dinámico y multifactorial. Las enfermeras son responsables de la gestión de la clínica, lo que implica la indicación de terapias, productos adecuados, evaluación y orientación profesional y consentimiento terapêutico.A terapia com bota de Unna exige do enfermeiro um comprometimento intenso relacionado ao saber técnico e científico, a fim de erradicar as possíveis complicações. Este estudo tem por objetivo relatar a vivência do Programa Cicatriza diante do cuidado à pessoa com úlcera venosa e as consequências iatrogênicas decorrentes do manejo inadequado da bota da Unna. A metodologia aplicada refere-se a um relato de experiência, com abordagem descritiva e qualitativa, desenvolvido por um programa de pesquisa e extensão do curso de graduação em enfermagem da Universidade Federal de São João Del Rei - Campus Centro Oeste, que realiza atendimentos às pessoas com lesões cutâneas no ambulatório universitário de Estomaterapia. Em 2021, a equipe Cicatriza se deparou com o caso de um paciente com insuficiência venosa, apresentando uma lesão iatrogênica caracterizada pela exposição de tendões, no membro inferior direito (MID). A terapia compressiva é considerada a principal alternativa para o tratamento de úlceras venosas, contudo, se não for aplicada corretamente, os malefícios se sobressaem aos benefícios, acarretando um tratamento não eficaz e aumentando as taxas de recidivas, o custo do tratamento, a quantidade de exsudato e consequentemente os riscos de complicações severas. Portanto, diante a vivência do caso evidencia-se que o êxito no tratamento das úlceras venosas é complexo, dinâmico e multifatorial.
